Pre-assessment: Eight pre-assessment questions are provided to guide your preparation for the assessment. We strongly encourage you to review the learning resources and PowerPoint suggested in the learning path. Assessment directions: For this assessment, consider yourself a public/community health nurse tasked with helping the county health department choose health priorities. First, you will conduct a health assessment of a community (referred to as an EPI-Table or Epidemiological Table). . We have provided the data needed for four counties surrounding the UW-Milwaukee College of Nursing. To use the data that we have provided, you should select one community from the following: Milwaukee County, Washington County, Racine County or Kenosha County to use the data we have provided. If you wish to provide data for a different county near your home or workplace, you will need to find that data on your own as the epidemiology table is required before completing the Community Health Assessment Paper.
Write a professional report of your findings for the county health department. There are two tasks to this assessment, first to complete the epidemiology table (EPI-table) and second, to write a professional report. You will attach your “EPI-table” as appendix 1 (after your reference list) to your report. You MUST download one of the four templates provided and type your information into the template. (Note – As noted above, if you wish to use a different county than one that is provided, you will need to fill in the epidemiological report data for that county).
