This written assignment will take the form of a case study of a patient whose clinical management you have recently been involved with.
The focus of this assignment is to demonstrate your knowledge and in-depth understanding of the management of people with chronic conditions, whilst critically discussing the implications for vulnerable groups.
You must also demonstrate an in-depth understanding of communication and clinical assessment skills within consultations by critically discussing how this would be undertaken for your patient, supported by research evidence from the literature.
Students will provide an in-depth appraisal, demonstrating excellent critical reasoning skills. Focused and justified recommendations will be made as to how problems/weaknesses identified in the plan may be overcome, and sustainability improved. Extensive research demonstrating use of a wide range of current secondary research sources will be evident. Academic style and referencing will be excellent.
