PLANNING
• Responding:
o How will the problem(s) be solved?
o What are the goals of patient care?
o What evidence–based information will you include in planning the care of your patient?
o Write the nursing and collaborative plans of intervention that address the problem(s) to achieve the goals of care.
IMPLEMENTATION
• Responding: Implement the plan of care

o What were the outcomes of the interventions?
o Were the goals met?
o Were the problems resolved?
