PM Respiratory therapist driven protocols (RTDP) allows proper assessment by a respiratory therapist to initiate or adjust treatments within a physician’s guideline. The thought plan behind a RTDP is to give more autonomy to the trained practitioner through a governing body of medical professionals. More times than not there are situations that involves patients with deteriorating ventilatory status in which medical services were delayed due to no protocol directing services. Cost effectiveness of therapies and time management can be alleviated with a working established protocol. An important aspect of the protocol is that major clinical changes will still be conducted through the physician but minor therapy adjustments can be made by the therapist. This simple but import aspect will also decrease the unnecessary need for calls to the physician for therapeutic changes. This makes it necessary for the physician to always be in the loop for any major changes in patient status that warrant intervention. As Respiratory therapists, we are trained to recognize abnormalities and how to alleviate exacerbations. A proper protocol is one that serves the purpose of a procedure that a task must follow and that is respiratory driven that shows effectiveness is supported by the AARC as a proper implemented RTDP.

Therapist Driven Protocols Policy The American Association for Respiratory Care (AARC) defines respiratory care protocols as initiation or modification of a patient care plan following a predetermined structured set of physician orders, instructions, or interventions in which the therapist is allowed to initiate, discontinue, refine, transition, or restart therapy as the patient’s medical condition dictates.
Most would agree that protocols provide a means to provide quality respiratory services; ensure that resources are consumed in only the provision of necessary care; minimize complications and cost associated with variability; bridge the relationship between physicians, nurses, and respiratory therapists; and reduce expenses for both the hospital and the patient.
