Mission for the facility.

Demographics of the facility.

  • Third paragraph:

The needs of the surrounding community.  Here you include the demographics of the neighborhood or borough, depending on the information you search for.

 GOAL OF THE NEEDS ASSESSMENT

  • To understand the facility, the services it provides, and the community it supports.

In regards to your facility:   Get an idea of who they serve: age, gender, race/ethnicity, education level, literacy, disease conditions, length of stay, and the effects of COVID-19 on clients and services.

Broader community: Target the community’s “state of nutritional health.” Use existing vital statistics data (CDC.gov)Seek the opinions of target member groups

Above often used to develop community programs addressing certain problem areas. It would be interesting to see what information is posted for COVID-19 morbidity and mortality.
