Produce a nursing care plan by selecting a clinical scenario encountered. The scenario from primary setting is based on an individual’s fundamental health and social care needs. There will be a requirement to demonstrate how effective communication can facilitate care planning and how the nurse works within a team to achieve person-centred care. You will need to explore relevant care planning methods, bundles and/or care pathways, care evaluation and develop a plan of care based on the patients’ fundamental care needs. There is also a need consider activities of living, how effective care planning and team working can optimise the individual’s functioning and wellbeing.
