1. A Critical Reflective Account (1000 words) in relation to how you have developed and progressed your underpinning current knowledge and skills within practice that
demonstrates your experiential progression from SCQF level 9 to SCQF Level 11.
2. A Case Study (1500 words) that includes evidence from your current practice which demonstrates your experiential progression to SCQF level 11 practice.
The reflective account and case study must be mapped to the relevant module learning outcomes.
1. Assess and take an appropriate medical, social and medication history using a biopsychosocial approach to include allergies and intolerances congruent with their context of practice.
2. Work with patients/carers and the multi professional team across care settings in partnership to make informed choices, agreeing a plan that respects patient preferences including their right to refuse or limit treatment.
3. Determine and select both non-pharmacological (including no treatment) and pharmacological approaches to modifying disease and promoting health.
4. Prescribe a medicine within their own scope of practice only with adequate, up-to-date awareness of its actions, indications, dose, contraindications, interactions, cautions, and unwanted effects.
5. Be able to critically reflect on examples of prescribing practice and apply this learning to enhance practice.
