People (Demographic, Biological Factors, Social Factors, and Cultural Factors) 2. Place/Environment (Physical Factors, Environmental Factor) 3. Social Systems (Health Systems, Economic Factors, Resources and Services) Research and Analysis of the Community • Data Collection (Conduct a Windshield survey) • Analysis of Community Assessment Data (Identifying Community needs and strengths) • Community Health Program (Identify a resource that this community could utilize to help alleviate the needs identified) • Summarize how your service-learning activity helps meet the Healthy People 2020 initiatives. • Reflect on what was learned from the assignment and how it will impact your practice in the future.
