Explain how you can use the information from the SWOT analysis as a health professional in your community to mitigate health disparities.

Use the Community Guide to locate an additional evidence-based program that is either similar to the one that you are focusing on or that addresses the same health disparity. Use the Resource Inventory as a guide.

Describe at least two similarities between the local health program and the evidence-based program from the literature.

A SWOT analysis is a tool used in gathering and identifying resources of a community, along with strengths and weaknesses of the environment. Such an analysis provides an organized and systematic way of gathering the information collected from a community. This, in turn, helps determine the areas of possible changes and explore new and alternative possibilities to solutions of issues within a community.

Review the resources that address SWOT analyses and perform a SWOT analysis on your local health program and vulnerable population. The Assessment Template contains general questions that can help you.

What does this type of data collection mean to me in my current or future role as a clinician/administrator?

Hospital comparison and data collection

Visit the website (Links to an external site.). Work your way down to the ‘Hospital Compare’ tool and query performance outcomes for at least 3 hospitals. You may wish to view the hospitals which employ you or at which you’ve been a patient. It might be interesting to view hospitals which are competitors. In reviewing this data, please respond to the following questions.

1. What does this type of data collection mean to me in my current or future role as a clinician/administrator?
2. What does this type of data collection mean to me as a consumer of healthcare?3. Do you anticipate reduced costs because of incentivized reimbursement for these measures? What are some ways that hospitals might “game the system”?

What are clinical quality measures (CQM)? Electronic Clinical Quality Measures (Links to an external site.)

Pulling structured data points from the electronic health record and reporting that data to a government entity such as the Centers for Medicare and Medicaid (CMS) has become a common practice over the last few years.  CMS has designed the specifics for reporting this data into several measures that are referred to as eCQMs (electronic clinical quality measure).
eCQMs help a hospital report on data such as ensuring a patient receives stroke education after experiencing a stroke for the first time or reporting the successful prescribing of an anti-clotting medication to a patient that has experienced a clot.  eCQMs will become an increasingly common method of pulling data out of electronic health records that will be used for hospital reporting purposes.  This assignment will help you explore the world of the eCQM and understand the important role this type of data monitoring will have on hospitals today and in the near future.

Instructions
Using the links provided, please answer these three questions (use the template above for Mod 4.2):
What are clinical quality measures (CQM)? Electronic Clinical Quality Measures (Links to an external site.)

How are eCQMs developed/structured? (HINT:  You may need to use some additional resources on the Web to answer this question…)

Choose one measure and briefly summarize the details of that eCQM.  Click this link (Links to an external site.) and scroll down to see a couple of options to choose from (e.g., Anti-Depressants, Breast Cancer Screening, Childhood Immunizations, etc).  Once you choose a measure, provide a summary of the measure on your template.  We’re looking for a summary of the measure description, the population being measured, who is excluded from this measurement, and who is included (numerator).
