Create a new org chart that is a logical and effective way to organize the full staff (existing and new). Consider that although the managing physician is responsible for any clinical aspects of all of the physicians, it is common for physicians to report to a practice administrator for all non-clinical aspects of their job (payroll, benefits, scheduling, issues between two employees, attendance, etc).

Healthcare organizations

For this assignment, there are 2 parts, combined into one assignment submission.

Part 1
Research organizational charts used in healthcare organizations. For example, see the org charts for Johns Hopkins:

https://www.google.com/search?sa=X&source=univ&tbm=isch&q=johns+hopkins+organizational+chart&client=firefox-b-1-d&ved=2ahUKEwjWiZKrtc_qAhWQVN8KHVZhBuYQsAR6BAgBEAE&biw=1366&bih=604

Now, assume the managing physician asked you to make a new organization chart containing all of the current staff that includes the managing physician, practice administrator, medical secretary, an office manager who oversees 2 coder/billers, a medical scribe, a medical receptionist, and a medical records clerk. The current practice also includes an ARNP clinical supervisor who oversees a PA, RN, 2 clinical medical assistants, and a radiology technician. The new org chart should include a full-service laboratory with a lab technologist and three lab technicians plus an imaging center for which they’ll hire a radiologist (a radiologist is a physician) and 3 more (you already have one) radiology technicians.

Use your own judgment – there is more than one acceptable way to do this assignment but it must be logical and effective as far as which positions report to the same supervisor and who reports to the practice administrator directly.

Part 2
Review Chapter 5 and do an Internet Search on short-term planning and long-term (strategic) planning. Specifically, look for planning for a health care facility. Based on your readings and your internet search, list and describe 3 resources, processes, tools, or strategies for long-term (strategic) planning and 3 different resources, processes, tools, or strategies for short-term medical office planning.

For example, a SWOT (Strengths, Weaknesses, Opportunities, and Threats) analysis is an example of a strategic/long-term planning process in which the top-level management conducts an environmental scan to determine the best approach to competing in their service area. An example of a short-term plan is an annual budget which guides each department in planning by providing a guide to how much they have to spend that year to achieve the goals for projects and productivity.

How has the two-midnight rule impacted the design and operations of healthcare? How effective has this policy been in curbing costs? Has it influenced what types of delivery structures healthcare systems are developing?

Operational and financial viability

Managers take two approaches to this challenge. Try to save your way to success by being frugal, efficient, and effective. And to market to available sources of revenue, e.g., insurance, self-pay, and retail outlets. It is not an either-or as some may believe. You can’t save your way to success.

Over the past 15 years, trends are moving to more “retail medicine.” See attached PowerPoint while at the same time maintaining a steady, predictable income stream from insurers and other third-party payers. Hospitals are becoming a focus for the very ill or injured, while outpatient treatment and surgeries are more cost-effective and popular with patients. The Medicare “two-midnight” rule has been a key driver as to whether care is delivered inpatient or outpatient, and the Medicare rules are a key driver/influence as to how insurance companies design their products.

Medicare two-midnight rule

This rule established a Medicare payment policy regarding the benchmark criteria that should be used when determining whether inpatient admission is reasonable and payable under Medicare Part A. … Medicare Part A payment is generally not appropriate for hospital stays not expected to span at least two midnights.

What are the impacts on your life if you could not walk more than a distance of 50 feet? What modifications would you have to make in your home and work environments to maintain your independence? What would happen if you were unable to make those modifications?

Week 4 Discussion

Respond to the following in a minimum of 175 words:

What are the impacts on your life if you could not walk more than a distance of 50 feet?
What modifications would you have to make in your home and work environments to maintain your independence?
What would happen if you were unable to make those modifications?

Identify all keys in each data table when applicable. List DBDL for each of your tables using the format described in the “Database Design Language (DBDL)” section (p. 181) in Chapter 6 of Concepts of Database Management.

Database Design Language (DBDL).

In the design process, there must be a mechanism to represent tables and keys. The shorthand notation that you have used so far represents tables and primary keys but it does not represent other related keys. An approach to represent tables and keys including primary, alternate, secondary, and foreign keys is Database Design Language (DBDL).
In Module Four, you selected a topic for your Final Project and began designing your database. As of now, the tables should be normalized and not contain update anomalies. Depending on the business model or assumption, check what type of relationships the tables have. If one-to-one, treat it as one-to-many. If one-to-many, no need to change anything. If many-to-many, create a new table to handle the many-to-many relationship.

In this assignment, you will present your tables using database design language (DBDL). On a Word document:
Identify all keys in each data table (i.e., primary key, alternate key, secondary key, and foreign key) when applicable.
List DBDL for each of your tables using the format described in the “Database Design Language (DBDL)” section (p. 181) in Chapter 6 of Concepts of Database Management. For an example, review Figure 6-1.

What is NDNQI and how was it used in the study by Garrard et al.? What is the primary key for the Patient Table? What is the foreign key for the Patient Table? What is the primary key for the Fall Table? Assume that each fall code can be used for multiple patients and each patient can receive only one fall code. What kind of relationship exists between the Fall Table and Patient Table?

NDNQI Injury Falls

For this assignment, refer to the article Reliability and Validity of the NDNQI Injury Falls Measure by Garrard et al. NDNQI fall measures provide a proper method for reporting injury falls, which could be useful for hospitals in comparing their data with reliable national comparison data to develop fall prevention plans.
Based on information presented in Table 1 (Garrard et al., pp. 116-117), an example of data tables that can be created is as follows:

Patient Table
PatientlD  LastName  FirstName  FallCode

001            Smith           Jane           2

002            Doe               John          2

003           Johnson        Mike         1

Fall Table
FallCode     Description

1                 None

2                Minor

3               Moderate

4                Major

5                Death

After reviewing this information, answer the following questions in a Word document:

  • What is NDNQI and how was it used in the study by Garrard et al.? Refer to the Module 6 Readings. A short paragraph is expected.
  • What is the primary key for the Patient Table?
  • What is the foreign key for the Patient Table?
  • What is the primary key for the Fall Table?
  • Assume that each fall code can be used for multiple patients and each patient can receive only one fall code. What kind of relationship exists between the Fall Table and Patient Table?
