Stake holder analysis

The stakeholder analysis (SHA) is an exercise to understand both the formal and informal project stakeholders. This analysis is usually done as part of your project charter and scope and you may have already completed some work on this. As we move towards the end of your project plan build, it bears repeating and reanalyzing to ensure you have captured the stakeholders’ expectations.
Managing change is a critical activity of any system implementation and perceptions change through the course any project. Often what is understood as expectations for stakeholders at the outset of a project is different at the project conclusion. Ongoing “check ins” with stakeholders are key to managing expectations (and change) that is inherent with large system migrations (in this case paper to digital charting with an EHR).

CASE STUDY:
The Waverly Family Health services has decided to implement an electronic health record (EHR). They have decided to implement a web-based EHR called Practice Fusion http://www.practicefusion.com. You will need to review the website to understand about the product. You may sign up for free access to their product to learn more and answer questions you might have about the product (see also provided youtube videos about Practice Fusion).
The clinic has no prior experience implementing an EHR, but they feel assured they can accomplish a web install and system conversion (migrating from paper charts to digital content) using the skill of their staff and providers. Hardware is already provided. The following is baseline information on the clinic and project resource allocation:

Budget
The clinic acquired a small business development loan on their American Express Card business account in the amount of $40,000.00 for the entire project. This includes all startup costs, labor, training, and “unforeseen costs.”

The following is a breakdown of staff who will need EHR access and new hardware to run the EHR:

Environment Assessment Information:
The clinic has a high speed T-line, which provides internet access with Wi-Fi access throughout the clinic. There is ample hardware throughout the clinic. Each clinic exam room (there are 4) has a workstation consisting of a Dell “all-in-one” desktop with 8GB of ram and Intel i7 processor, and a 23-inch screen. The units are wall mounted and the monitor is on an articulated arm allowing the patient to see the screen when the clinician wants to share information. Each of the Medical Assistants (MAs), front office clerk, biller and directors have similar work stations. The workstation configurations meet the minimum standards for utilizing the web-based EHR.

1. The ethical questions of Informed Consent are far reaching, more than I ever thought.

To review, one of the 4 goals of AI in healthcare is Informed Consent, here is the list again:

I found this FANTASTIC article (https://papers.ssrn.com/sol3/papers.cfm?abstract_id=3529576) that runs through a scenario of a patient speaking with his physician and the recommended course of treatment for his diagnosis. Please take just 5 minutes to read this. The article brings up a host of ethical questions about informed consent and asks some very hard to answer questions.

The last question is very poignant, “If you were to find out that the AI/ML system was used to make recommendations on your care and no one told you, how would you feel? Well, come to think of it, do you know whether an AI/ML system was used the last time you saw a physician?”

2. Cloud is a fancy term for “someone else’s computer”. Rather than running application servers and storing data on premises, it is transmitted across a secure Internet connection to the cloud location. This model is becoming very popular, it saves the practice/organization from purchasing expensive computers/servers and hiring expensive IT staff who would normally perform the maintenance and support of the servers running the applications and storing the data.

There are many different types of applications that can be offered up from the cloud to a practice/organization, from EHR applications to organizations who analyze large sums of data to support their AI and decision support models.

Our learning activities, “How does IBM Watson work?” (https://www.youtube.com/watch?v=r7E1TJ1HtM0) and “Watson Health for Diabetes Management” (https://www.youtube.com/watch?v=KFD8HFqNR18) describe how wearables, data management and analytics in the Watson Health Cloud. IBM is then able to make predictive analytics for a give patient and sends information/recommendations to the patient’s smartphone. IBM claims they can predict if a patient is on their way to experiencing a diabetic event and send the patient information and recommendations to avoid the predicted event.

I don’t think this is replacing healthcare practitioners, but what are your thoughts on this type of predictive analysis using the cloud?

The Patient Protection Affordable Care Act (PPACA) was an act that was created in attempt for the reform of healthcare under the Obama administration. Some of the bills that were introduced included an increase in access to health care, an increase in primary care workers, and services for promotion of health and to prevent diseases (Goudreau & Smolenski, 2018). This bill was a process and had revisions made under the House and the Senate in order for an agreement to be made (Goudreau & Smolenski, 2018). There were provisions in the act that were beneficial for providers, especially advanced practice registered nurses.

             One provision that was beneficial for nurse practitioners were that it protected the patient’s choice and allows the patient to choose what provider they see, and payment nondisclosures were implemented as well.
