Benchmark- Capstone Project Change Proposal

In this assignment, students will pull together the capstone project change proposal components they have been working on throughout the course to create a proposal inclusive of sections for each content focus area in the course. For this project, the student will apply evidence-based research steps and processes required as the foundation to address a clinically oriented problem or issue in future practice.

Develop a 2,500-4,000 written project that includes the following information as it applies to the problem, issue, suggestion, initiative, or educational need profiled in the capstone change proposal:

Background
Clinical problem statement.
Purpose of the change proposal in relation to providing patient care in the changing health care system.
PICOT question.
Literature search strategy employed.
Evaluation of the literature.
Applicable change or nursing theory utilized.
Proposed implementation plan with outcome measures.
Discussion of how evidence-based practice was used in creating the intervention plan.
Plan for evaluating the proposed nursing intervention.
Identification of potential barriers to plan implementation, and a discussion of how these could be overcome.
Appendix section, for evaluation tools and educational materials, etc. are created.

Review the feedback from your instructor on the PICOT Question Paper, and Literature Review. Use this feedback to make appropriate revisions to these before submitting.

Hello Class,

The Nursing Profession is a caring and compassionate profession, however, it can be complex at times.  Within my years of being a nurse I definitely utilized all of the professional core values. The values that truly serve me as a nurse are honesty, integrity and patient equality.  I strive to understand my patients to better serve them and their needs but there was a time when I believed that I went against those values.

A challenging experience for me was having a trans gender patient. The experience was difficult because I have never had a sick transgendered patient where I had to provide care for. I usually do outreach and health fairs that support the LGBT community however, this was my first transgendered patient on the job. Before meeting the patient, I received report that this patient was difficult and verbally abusive towards the staff and I automatically thought to myself that I would have a rough day.

I went into the room with a positive attitude and introduced myself. I asked the patient how to address them and she replied “as she, her, I am a woman”. I then asked the patient if I could perform my head to toe assessment. She said “yes”. She also said that, “you are the first person that asked me how to address me and did not just call me a man” .  She was impressed about the way  I made her feel comfortable. This experience changed because I was able to learn from this person and also confront my own challenges.  I believe that myself and the other staff did not treat this patient with the equality she deserved.  We did not consider the fact that maybe she needed more compassion in order to overcome her challenges.

With continuous training and exploring certain topics, challenges in the nursing profession can start to be broken down. Care can continue to be provided effectively and the atmosphere can be changed to a more positive one.

Hello everyone, -CP

Nurses are expected to be responsible, honest, have integrity, treat every patient with equality and have the desire to prevent and alleviate suffering. Nursing is a profession that is rooted in professional ethics and ethical values, and performance is based on such values. Nursing is ranked as one of the most ethical and honest professions. Our values are goals and beliefs that establish behavior and provide a basis for our decision making. As nurses we face many day-to-day problems and complex decisions each day.

Nursing is both challenging and rewarding at the same time. With that being said, we are held to a high standard of professionalism. Recently, I encountered a challenging situation at work with a charge nurse. I recently switched departments in my hospital and ever since I transferred to the Emergency Department, I felt some sort of hatred towards me by one of my charge nurses. I was helping my colleagues in the Resuscitation Bay, and I felt the charge nurse following me around. She finally came up to me and scolded me that I was not working and that I am not allowed to just do what I want. At that very moment I felt embarrassed and looked down upon me. There were many nurses and doctors around and I felt that what she did was extremely unprofessional. I was very frustrated and angry because this could have been resolved in a private room or space. I asked her if we could speak in a private room because I did not respect how the situation was handled. We went into a private room to talk but it did not end well. For weeks she did not acknowledge or speak to me. Whenever she needed to deliver a message to me, she would have someone else speak to me. It was extremely uncomfortable for me to deal with this every time I worked with her. A workplace environment should never be this way. This situation made me truly realize that respect and equality are critical core values in the nursing profession.

We come across people from all walks of life daily. I believe that no matter where we come from, we should all be treated with the same equality and respect. With respect, if everyone is treated equally and with respect for one another then everyone should be able to work together, solve problems, and feel cared for. Giving respect to every worker provides a better work atmosphere that helps create a conducive work environment where employees feel safe and happy.

Explain how all four terms in your example work together to form the basis for a testable theory. Identify a second nursing theory and provide a general overview of the theory.

Theoretical Foundations

Theories are structured ideas attempting to explain a specific thing. This week’s reading introduces the ideas of phenomena, concepts, propositions, and assumptions. This assignment is designed to help you think about those four aspects and explain the connections between each to help strengthen your understanding of the foundations and working parts of theory

Define the following four terms in your own words, citing at least one source: phenomena, concept, proposition, and assumptions.

Review your definitions and think of an example from your day-to-day life or your practice experience that includes each of the four terms.

Present your example from day-to-day life or your practice experience that includes all four terms.
Explain how all four terms in your example work together to form the basis for a testable theory.
Identify a second nursing theory and provide a general overview of the theory.
Explain how that theory is applied in your current clinical practice.

How will PoC device integration prevent future errors from occurring with the wrong breast milk given to the wrong baby? Why will human factors science and adoption science be important considerations for this project team?

Device Integration Improving Patient Safety

CASE STUDY

A large healthcare system with a neonatal ICU (NICU) has experienced a serious patient safety error with the wrong mother’s breast milk given to a neonate. The patient safety and quality improvement (QI) department evaluates the incident, evaluates studies that address safe practices with NICU, and plans a QI project to address the event. In examining best practices nationally, they discover that bar code medication administration (BCMA) is being applied to administration of mothers expressed breast milk. The QI department solicits the expertise of the Chief Nursing Informatics Officer (CNIO). The CNIO assigns an informatics nurse specialist to the project to implement device integration at the point of care (PoC) to prevent any future errors from occurring and follow national best practices for using BCMA to administer mothers expressed breast milk to NICU infants.

1. How will PoC device integration prevent future errors from occurring with the wrong breast milk given to the wrong baby?
2. Why will human factors science and adoption science be important considerations for this project team?
3. Why did the quality department call in the CNIO to help with this project?
4. How will the project team need to consider integrating the data to and from the EHR through an interface for PoC BCMA, and why is this important?

Consider the results presented in the SPSS output and reflect on how you might interpret the frequency distributions and the descriptive statistics presented.

Frequency and Descriptive Statistics

Consider the results presented in the SPSS output and reflect on how you might interpret the frequency distributions and the descriptive statistics presented.

The Assignment:

Summarize your interpretation of the frequency data provided in the output for respondent’s age, highest school grade completed, and family income from prior month.

Note: A frequency analysis is way of summarizing data by depicting the number of times a data value occurs in the data table or output. It is used to analyze the data set including where the data are concentrated or clustered, the range of values, observation of extreme values, and to determine intervals for analysis that could make sense in categorizing your variable values.

Summarize your interpretation of the descriptive statistics provided in the output for respondent’s age, highest school grade completed, race and ethnicity, currently employed, and family income from prior month.
