An important element of delivery of nursing care is the professional practice model (PPM)within which it is framed. These models define the structures and processes within which care is given. It can delineate the degree of autonomy within which nurses can practice that leads to higher quality of care. While these frameworks are informed by nursing, organizations must support implementing these models.

Regardless of the type of facility, nursing practice should be framed within a specific nursing professional practice model. Most PPM have components that include the nursing vision/values, nursing practice standards, teamwork, recognition and advancement, evidence-based practice/research, nursing care delivery model, shared decision-making, and some type of reflective practice.

Purpose
The purpose of this assignment is to help you learn to identify roles and responsibilities of the advanced registered nurse practitioner as a member of the health care team.

Overview
In this multi-week assignment, everyone in the class will be required to research 2 professional health care roles, share their research, and draw on the shared research to create a brief outline of a collaborative, evidenced-based plan for the patient described in the case Interprofessional Simulation. To make this collaborative, we will do the following:

Each person will be assigned by the instructor to “play the part” of one of the health care professionals listed in the case and shown below:

  • Health care administrator
  • Mental health practitioner (Links to an external site.)
  • Nutritionist
  • Occupational therapist (Links to an external site.)
  • Physical therapist (Links to an external site.)
  • Physician assistant (Links to an external site.)

Each person will then research the scope of practice for that profession as it pertains to the case and the practice act (if applicable) for the given state (Florida). If the role is not mentioned in the practice act (and no link is given above), then you should consult other resources you find. Reach out to the instructor for guidance as needed (research to be done in Modules 2–4).
Each person will also research the part of the nurse practitioner (Links to an external site.) (research to be done in Modules 2–4).
Each person will post their research for the assigned role (not nurse practitioner) for the other class members to review and draw upon (post at end of Module 4).
In addition to the nurse practitioner, select 4 professions you would want to be on the team to create a collaborative health care plan for the patient. You do not have to include the role you researched. However, you must justify your choices of health care providers beyond the nurse practitioner who will contribute to this plan. Using research provided by classmates on other roles, and, as applicable, the research you did on your assigned role, as well as research you did on the nurse practitioner role, you will individually and independently create and post a brief outline for a collaborative, evidenced-based plan for the patient. For details about the plan, see the Interprofessional Simulation case description and Discussion 6. Complete the plan and post it by the Discussion 6 due date.

Note: This is not a traditional group project where you work together with a few other class members to make one plan for the patient as would normally be done in professional practice. Rather, you will each share your research so that you can each make your own plan for the patient taking into consideration the roles that other health care professionals could or should assume in the case. The reason for doing it this ways is that, while a real life situation would require health care professionals to work together to create one plan, there are too many obstacles for such a group project in a 6-week online course. Moreover, with individual work, you will be able to compare your plan with those done by your classmates and learn from the ideas they share.

Purpose
The intent of this assignment is to help you identify and apply health promotion and prevention in primary care across the lifespan.

Overview
The Family Genogram
A family genogram captures information about family structure, major life events, illnesses related to genetics and lifestyle, and family relationships. This activity is useful when a clinical situation suggests that a family assessment is warranted – specifically, a person’s risk for family-related illness or stressors such as diabetes, hypertension, heart disease, alcohol and/or substance abuse, and depression (Turabian, 2017). Family has an impact on the development of personality, including attitudes, behaviors, and beliefs. While a person can change over the course of their life, the influence of family cannot be avoided.

Skills for dealing with an individual and their family are paramount in the primary care process. Whenever possible, we want to include the family – the nuclear family and the annexes (spouse, in-laws, friends, and community) – in the treatment plan (Turabian & Franco, 2016). In today’s health care environment, we call this family-centered care (Clay & Parsh, 2016).

The genogram will include the primary family and at least 2 previous generations, if appropriate, to see health patterns including ages of family members, causes of death, occupations, marriages, divorces, and children. The genogram is a great way to “break the ice” with the family and become engaged with them.

The genogram includes the following:

  • Primary family composition: parents, children, dates of birth, dates of death, occupations, and any health issues stated (or indicate none).
  • Parents (of the primary family composition), siblings, and other related family members who may have a significant health issue. Include dates of birth, death, health issues (stated or indicate none).
  • Grandparents: dates of birth and death, causes of death, and any health issues stated (or indicate none).

The Ecomap
An ecomap shows the social and personal relationships of an individual with their environment. This tool documents connections between family members and the quality of their relationships either as positive and nurturing or causing conflict and stress. An ecomap can assist the health care provider in identifying potential family members who are sources of depression, stress, and/or anxiety. It also helps determine support systems in friends, neighbors, clubs, social and religious organizations and more (Lynch, Keating & MacFadyen, 2014; Ohio University Medicine, nd).

The ecomap will give you a visual of the complexity of family situations by seeing the various connections to the community, time spent in activities of daily living outside the home and influenced by these connections on the family. This provides a visual for families to see “all they do” and “all they are involved in,” and perhaps acknowledge why they are tired or stressed.

The ecomap includes the following:

  • Primary family composition: parents, children and/or other household members.
  • Various activities in the community including school, work, places of worship, leisure and volunteer activities, family visitations, etc.
  • Time spent per week at these activities.

Interview
For this part of the portfolio assignment, you will hold your first interview with a family that has a health concern or problem. The purpose will be to gather information to create a genogram and ecomap for the family. This family should be from a clinical site that you have access to in the community. You may choose a neighbor, acquaintance, or co-worker’s family for an interview and assessment. This activity will give you a visible picture of the complexity of the family and provide patterns of health through three generations.

Note: do not choose your close friends or a family you are close to (including your own), because this closeness may create a bias and lack of objectivity.

While constructing the genogram and ecomap with the members of the family you have selected (who will be future patients), use the time to note family interactions, communication patterns, and time spent in various activities. Again, this is the first part of your portfolio assignment. You will have an opportunity to obtain feedback prior to completing Part 2 of the portfolio assignment.
