What zip code did you survey? What would be a need for a community health nurse to begin a health promotion project in this community?

Windshield Results:

What zip code did you survey? 33127
What would be a need for a community health nurse to begin a health promotion project in this community?
o After going into the community, it was concluded that many of the residents on the north side of the zip code did not take advantage or participated in the public parks around the community.

There weren’t many residents observed walking or exercising in the community. A good nursing intervention would be to establish exercise activities in the parks to attract residents and encourage them to workout.
o A need for the community would be to have exercise programs implemented in the parks and the recreation centers. This will encourage community engagement and exercising for the residents.

Directions: This assignment follows the findings of the Windshield Assessment conducted previously. Take the nursing intervention that was recommended in your assessment and prepare a paper.

Assignment Criteria:
Use the MAPIT model to create the paper. Be sure to include a cover page and reference page.
This APA paper should include How the community nurse would plan for change using the following topics

Mobilize:
o What is the vision and mission of the coalition?
o Why do I want to bring people together?
o Who should be represented?
o Who are the potential partners (organizations and businesses) in my community?

Assess
o Who is affected and how?
o What resources do we have?
o What resources do we need?

Plan
o What is our goal?
o What do we need to do to reach our goal? Who will do it?
o How will we know when we have reached our goal?

Implement
o Are we following our plan?
o What can we do better?

Track.
o Are we evaluating our work?
o Did we follow the plan?
o What did we change?
o Did we reach our goal?

Utilize these sections as headers for your paper. This will help you to learn how to begin to plan and implement a small intervention of change at your assigned community site.
The paper should be between 23 pages (not including cover and reference page) and include at least 2 references. Be sure to use APA format.

Consider the past achievements in public health as many have implications around the globe. Describe a global health concern that has impacted your community or those you care for. Factors that will continue to impact community and population health

There are many factors that will continue to impact community and population health.

Consider the past achievements in public health as many have implications around the globe.
Describe a global health concern that has impacted your community or those you care for.
Discuss what surveillance data could be pulled from an electronic health record (EHR) you have used or currently use in your nursing practice.
Take a moment to review the Twitter feed for compelling Tweets that are related to informatics, global health, and population health outcomes.
Share an overview the Tweet and how it is likely to impact future collaborative trends in community health.
Compose a 280-character or fewer Tweet that describes what you would like to see as a Tweet in the next decade related to public health achievements. Please try to be realistic, but also be visionary. Remember Twitter only allows 280 characters (this includes spaces, etc.) so you will need to be concise. It should include a hashtag. Include a reference URL if applicable. Share this Tweet in the discussion.

(Note: A Twitter account is not required nor share your Tweet on social media).

Your discussion post should look like:
Paragraph one: Describe a global health concern that has impacted your community or those you care for
Paragraph two: Discuss what surveillance data could be pulled from an electronic health record (EHR) you have used or currently use in your nursing practice.
Paragraph three: Share information from a Tweet and how it is likely to impact future collaborative trends in community health.
Paragraph four: Compose a 280-character Tweet that describes what you would like to see as a Tweet in the next decade related to public health achievements.
Resources: Where did you find your data?

What is the health issue that is most prevalent or severe in the population? How does the health issue impact the daily lives of members of the population? How does the care environment in your chosen context impact both the population and the level of care related to the health issue? What are the biggest challenges that you would need to overcome to improve the outcomes for the population related to the health issue? What is your position on how to best work to improve the care and outcomes that the population is receiving?

Analysis of Position Papers for Vulnerable Populations

Develop a 4-6 page position about a specific health care issue as it relates to a target vulnerable population. Include an analysis of existing evidence and position papers to help support your position. Your analysis should also present and respond to one or more opposing viewpoints.

Introduction

Note: Each assessment in this course builds on the work you completed in the previous assessment. Therefore, you must complete the assessments in this course in the order in which they are presented.

Position papers are a method to evaluate the most current evidence and policies related to health care issues. They offer a way for researchers to explore the views of any number of organizations around a topic. This can help you to develop your own position and approach to care around a topic or issue.

This assessment will focus on analyzing position papers about an issue related to addiction, chronicity, emotional and mental health, genetics and genomics, or immunity. Many of these topics are quickly evolving as technology advances, or as we attempt to push past stigmas. For example, technology advances and DNA sequencing provide comprehensive information to allow treatment to become more targeted and effective for the individual. However as a result, nurses must be able to understand and teach patients about the impact of this information. With this great power comes concerns that patient conditions are protected in an ethical and compassionate manner.

Position papers are a way for individuals, groups, and organizations to express their views and intentions toward a specific issue. In health care, many position papers address specific policies, regulations, or other approaches to care. As a master’s-prepared nurse, you should feel empowered to express and advocate for your own views on policy and care matters. This is especially important when it comes to populations you or your organization cares for that are not receiving the quality, type, or amount of care that they require.

An important skill in creating a position paper or policy proposal is the ability to analyze and synthesize others’ views about the population or issue of interest to you. By synthesizing the positive and negative views of an issue, you can become better equipped to strengthen your own arguments and to respond to opposing views in an informed and convincing way.

Preparation

As you prepare to complete this assessment, you may want to think about other related issues to deepen your understanding or broaden your viewpoint. You are encouraged to consider the questions below and discuss them with a fellow learner, a work associate, an interested friend, or a member of your professional community. Note that these questions are for your own development and exploration and do not need to be completed or submitted as part of your assessment.

    • What previously published position papers support your position, or the need to work to improve care and outcomes in general for the population?
      • How do these position papers support your assertions?
      • How could one or more of the position papers help you to form a treatment plan?
    • What previously published position papers contradict your position?
      • What, if any, of these differences would make your position stronger if you incorporated them?
      • How could you respond to any irreconcilable differences in such a way as to encourage buy-in for your position from those opposed?

What are the effects of controlling BP in people with diabetes? What is the target BP for patients with diabetes and hypertension? Which antihypertensive agents are recommended for patients with diabetes?

Discussion Question

L.N. is a 49-year-old white woman with a history of type 2 diabetes, obesity, hypertension, and migraine headaches. The patient was diagnosed with type 2 diabetes 9 years ago when she presented with mild polyuria and polydipsia. L.N. is 5′4″ and has always been on the large side, with her weight fluctuating between 165 and 185 lb.

Initial treatment for her diabetes consisted of an oral sulfonylurea with the rapid addition of metformin. Her diabetes has been under fair control with a most recent hemoglobin A1c of 7.4%.

Hypertension was diagnosed 5 years ago when blood pressure (BP) measured in the office was noted to be consistently elevated in the range of 160/90 mmHg on three occasions. L.N. was initially treated with lisinopril, starting at 10 mg daily and increasing to 20 mg daily, yet her BP control has fluctuated.

One year ago, microalbuminuria was detected on an annual urine screen, with 1,943 mg/dl of microalbumin identified on a spot urine sample. L.N. comes into the office today for her usual follow-up visit for diabetes. Physical examination reveals an obese woman with a BP of 154/86 mmHg and a pulse of 78 bpm.

Questions
What are the effects of controlling BP in people with diabetes?

What is the target BP for patients with diabetes and hypertension?

Which antihypertensive agents are recommended for patients with diabetes?

Describe the SBIRT process and use in healthcare. How was the Motivational Interviewing process applied and shared your client’s score and need for behavioral changes. Describe the referral for follow up treatment.

SBIRT approach to drug and alcohol use

This is an exercise combining screening for alcohol and harmful drug use, with a ‘brief intervention’ using Motivational Interviewing and referrals for follow up with behavioral health specialist.

For this assignment, create a narrated presentation regarding a patient (alcohol abuse) case scenario, using the Screening – with Brief Intervention and Referral to Treatment (SBIRT) approach.

1. SBIRT overview: Describe the SBIRT process and use in healthcare.

2. Case Presentation (alcoholic patient presents to ER with several complaints) Do not include patient identifying information but include age, chief complaint, and risk factors

3. Screening Tool used ( for example AUDIT). Describe tool, validity, scoring information and citation. Your patient’s score, and the interpretation of the patient’s score.

4. Brief Intervention: How was the Motivational Interviewing process applied and shared your client’s score and need for behavioral changes

5. Referral for Treatment: Describe the referral for follow up treatment

6. Evaluation of the process: Share patient outcome and provider evaluation of SBIRT process

7. References – 3-5 references. APA format. Include Screening Tool authors.
