Laud Humphreys’ Tearoom study is a good example of unethical research. In his study Humphrey observed clueless individuals engage in homosexual activities in various “tearooms”. Humphrey systematically observed the subjects of his research with an aim of understanding of the rules and roles, patterns of collective action, and risks of the game associated with impersonal gay sex in public restrooms (Babbie, 2004). According to Babbie, humphrey tracked down some of the subjects and interviewed them to gain a better understanding of their identity, lives, and rationale of engaging in homosexual activities in the said “tearoom”. This research not only violated the subject’s privacy but also didn’t give them the chance to agree or disagree to participate. Babbie (2004), explains that Humphrey research methods of using deception by not revealing himself to the subjects of his research was unethical. This is not simply a matter of weigh benefits vs risks because the outcomes of the research had nothing positive to offer the subjects. The subjects were not physically harmed but could have been potentially traumatized by the fact that someone was invading their privacy. Humphrey’s research violated the autonomy of the individuals who became part of Humphreys’ study without their knowledge.

The WMA Declaration of Helsinki developed in 1964 is a set of ethical principles regarding human experimentation and research for the medical community by the World Medical Association (WMA). This declaration is widely regarded as the cornerstone document on human research ethics. These principles offer guidelines and regulate research done on humans. According to the declaration of Helsinki informed consent should be put in writing after the subjects have been fully informed the details of the study (WMA, 2018). The declaration of Helsinki also states that it is the duty of researchers to protect the life, health, dignity, integrity, right to self-determination, privacy, and confidentiality of personal information of research subjects. The Belmont Report written by the National Commission for the Protection of Human Subjects of Biomedical and Behavioral Research is comprised of three basic principles that act as a framework for research involving human subjects. The three principles which are respect for persons, beneficence, and justice (The Belmont report, 2016). Humphrey tearoom study is a good example of an unethical study that did not consider the potential effects of the study on the unwilling participants and also did not give the subjects a chance to volunteer or opt out.

Review the Focused SOAP Note template, which you will use to complete this Assignment. There is also a Focused SOAP Note Exemplar provided as a guide for Assignment expectations.

Review the video, Case Study: Sherman Tremaine. You will use this case as the basis of this Assignment. In this video, a Walden faculty member is assessing a mock patient. The patient will be represented onscreen as an avatar.

Consider what history would be necessary to collect from this patient.

Consider what interview questions you would need to ask this patient.

The Assignment

Develop a focused SOAP note, including your differential diagnosis and critical-thinking process to formulate a primary diagnosis. Incorporate the following into your responses in the template:

Subjective: What details did the patient provide regarding their chief complaint and symptomology to derive your differential diagnosis? What is the duration and severity of their symptoms? How are their symptoms impacting their functioning in life?

Objective: What observations did you make during the psychiatric assessment?

Assessment: Discuss the patient’s mental status examination results. What were your differential diagnoses? Provide a minimum of three possible diagnoses with supporting evidence, and list them in order from highest priority to lowest priority. Compare the DSM-5 diagnostic criteria for each differential diagnosis and explain what DSM-5 criteria rules out the differential diagnosis to find an accurate diagnosis. Explain the critical-thinking process that led you to the primary diagnosis you selected. Include pertinent positives and pertinent negatives for the specific patient case.

Plan: What is your plan for psychotherapy? What is your plan for treatment and management, including alternative therapies? Include pharmacologic and nonpharmacologic treatments, alternative therapies, and follow-up parameters, as well as a rationale for this treatment and management plan. Also incorporate one health promotion activity and one patient education strategy.

Reflection notes: What would you do differently with this patient if you could conduct the session again? Discuss what your next intervention would be if you were able to follow up with this patient. Also include in your reflection a discussion related to legal/ethical considerations (demonstrate critical thinking beyond confidentiality and consent for treatment!), health promotion, and disease prevention, taking into consideration patient factors (such as age, ethnic group, etc.), PMH, and other risk factors (e.g., socioeconomic, cultural background, etc.).

Provide at least three evidence-based, peer-reviewed journal articles or evidenced-based guidelines that relate to this case to support your diagnostics and differential diagnoses. Be sure they are current (no more than 5 years old).

Identify two people, at two different levels (upstream, lateral, downstream) that you need to communicate with for your project and compare/contrast your communication with them based on each of the five rights of communication.

Collaboration Cafe: Communication in Project Management

Think about all of the stakeholders and colleagues that must be kept up to date on the status of your project. The recipient of your communication may be upstream (higher on the organizational chart), lateral (an equal organizationally), or downstream (i.e. end user) in relation to the project. A student in a prior CGE course compared communication in project management to the five rights of medication administration…we need to give:

The right person
The right information
(via) the right route
(at) the right time
(in) the right dose

Identify two people, at two different levels (upstream, lateral, downstream) that you need to communicate with for your project and compare/contrast your communication with them based on each of the five rights of communication.

One of the most unethical research projects known was the Nazi Medical Experiments. In the early to mid-1940’s doctors and their assistants forced participation into experiments that were not only evil but fatal at times. Some of the participants from Nazi concentration camps included Jews, Soviet prisoners of war, and Catholic priest, women and children were not spared. There were many experiments done and results were used to benefit German soldiers. Bone, muscle and nerve transplant, intentional poisoning and freezing experiments were just a few of the horrible test participants endured “Nazi experiments conducted on concentration camp inmates continue to serve as extreme examples of cruelty” (Carrothers , 2020, p.31). These experiments were done without anesthesia and if the participants did not die, they were often left disfigured or permanently disabled.

Because the participants were not willing, there was no informed consent. They were unaware of the purpose of the experiments or what they entailed. The Belmont Report was developed to support ethical research, it consist of three principles: beneficence, respect human dignity, and justice. These principles state there is a duty of the researchers to minimize harm to the participants, the research should be beneficial to the participants and others, and the participants have a right to full disclosure as well as the right to fair treatment (Polit & Beck, 2021. p. 62)

Code of ethics have been created due to corrupt and unprofessional research practices. People in vulnerable groups are at greater risk of being taken advantage of and need to be protected. In response to the Nazi Medical Experiments, the Nuremberg Code was created, it states “The voluntary consent of the human subject is absolutely essential, the person involved should have legal capacity to give consent should be so situated as to be able to exercise free power of choice, without the intervention of any element of force” (Christianto, 2020, p. 2). The Declaration of Helsinki was also created to protect research participants from inhumane treatment with such principles as making the participant priority and keeping them safe. With this declaration, participation is voluntary and the benefits outweigh risks. These codes and declarations keep research ethical, and participants are protected from deliberate harm and results help the participants and/or others.
