Identify the problem/issue (what is wrong with the current situation), g., increasing prevalence of a health condition and insufficient evidence for a solution;describe the background (what is the nature of the problem, or the context of the situation, that readers need to understand).

Covid-19 pandemic and mental health

One purpose of the research is to serve as evidence to address nursing problems that are important for nursing practice. For this assignment, you will choose a sub-topic of your group poster presentation topic and focus on a problem/issue that interests you. You will:

Identify the problem/issue (what is wrong with the current situation), g., increasing prevalence of a health condition and insufficient evidence for a solution;describe the background (what is the nature of the problem, or the context of the situation, that readers need to understand);

explain the scope of the problem (how big is the problem and how many people are affected);

describe the consequences of the problem (economic implications; the impact of the problem on population health; what will happen if we don’t address the issue); identify the knowledge gaps (what information about the problem is lacking)

describe the proposed solution (how will the new study contribute to the solution of the problem) and; indicate a research question that arises for you as you consider the literature related to the problem/issue and its significance (questions end in a question mark).

What questions does Orlando’s theory guide the nurse to consider in caring for Susan and Sam? Develop a family plan of care from the perspective of Orlando.

Orlando and the 1950s and 60s

Case study:
Ann, a community nurse, made an afternoon home visit with Susan and her father. After the death of her mother, Susan had growing concerns about her father living alone. “I worry about my father all the time. He is becoming more forgetful and he has trouble seeing. Mom used to take care of him. I am not sleeping and I am irritable around him. Yesterday I shouted at him because he wouldn’t let me help him with his laundry. I felt terrible! I am at my wits’ end! My brothers and sisters do not want to put dad in a nursing home but they are not willing to help out. As usual, they have left me with all the responsibility. I work part time and have two small children to care for.” Susan’s father, Sam, sat quietly with tears filling his eyes. He was well nourished and well-groomed but would not make eye contact. Nurse Ann noticed that the house was clean and orderly. A tray in front of the TV had the remains of a ham sandwich and glass of ice tea. Mail was piled up, unopened on a small table near the front door. There was only one car in the driveway and the yard was in need of attention.

Explore the 1950 and 60’s in the United States:

• Explore was happening in the United States during this time (culture, social, economics, struggles)

• What did nursing look like during this time (what were their jobs like, responsibilities, dress, autonomy, respect)

• What is the most influential accomplishment in nursing theory from the 1950’s and 1960’s?

Describe 2 ethical-legal dimensions and the nurse’s responsibilities related to the client. Explain 2 modifications based on client limitations that the nurse should make in order to complete a safe, effective, and comfortable physical exam.

Part 1:

Binita Wadhwa is a 72year old female who presents to the emergency department with left wrist pain after falling at home. She was diagnosed with mild cognitive impairment and diabetes mellitus type II in her 60s. She has a history of hypertension, transient ischemic attack, vision loss, renal insufficiency, and peripheral neuropathy. She has had many previous hospitalizations related to diabetic complications and cardiovascular
complications. She sometimes forgets to take her medications and does not adhere to diet or fluid restrictions. She states “There are few things left I can enjoy so I eat what I want when I want. I just want to be left alone”. She smoked for much of her adult life, but quit in her 50s.
She is essentially wheelchair bound and on supplemental oxygen via nasal cannula. Her primary caregivers are her daughter and soninlaw. Further interviewing reveals that she is left at home alone for long periods of time while they work; and she injured her wrist in the kitchen after falling while trying to get something to eat. Upon inspection; she appears slightly unwashed, smells faintly of urine, and has multiple ecchymoses and skin tears visible on both arms in various stages of healing. She is thin and gets short of breath easily with excessive talking and movement. She is a follower of Hinduism. For Ms. Wadwha,

1. Describe 2 ethicallegal dimensions and the nurse’s responsibilities related to the client (10 points)

2. Explain 2 modifications based on client limitations that the nurse should make in order to complete a safe, effective, and comfortable physical exam (10 points)

3. Discuss 2 potential healthcare referrals for the client and the need/benefit to the client (10 points)

4. Explain 2 potential cultural and/or spiritual considerations related to care for this client (10 points)

Part 2:

Find an adult volunteer with sibling(s) who is aware of his/her biological family’s health and complete afamily history. Use essay format or place information in a table. A blank table is available to copy/paste. For
the volunteer,

5. Complete a history of 3 generations. For each family member, identify initials, gender, alive/deceased, current age/age at death, major illnesses, and cause of death. If unknown, state what is unknown (20 points)

6. Describe patterns of illnesses and genetic health risks based on the family history (25 points)

Describe what you thought about the situation (e.g. its cause, potential resolutions, patterns you noticed). Describe any similar situations you have encountered in practice before. Describe any similarities and differences you observed when compared with the current situation.

Reflective Journal Assignment

Instructions: Choose a specific situation from clinical for your journal assignment. The situation can be a specific physiological patient problem, such as an elevation in temperature, respiratory difficulty, or electrolyte imbalance; or a situation involving a patient’s family. The situation can be a description of your role in interdisciplinary problem solving or an ethical issue you encountered.  Although there are many ways of organizing your thinking about patient care and professional nursing practice, Tanner’s (2006) Clinical Judgment Model provides the framework for the questions in this journal assignment.

Introduction/Background: Describe a nursing situation you encountered during clinical. Describe your relationship to the patient at the time you noticed the situation (e.g., previous contact with patient and/or family, the quality of your relationship). Consider experiences you have had that helped you provide nursing care in the situation. Describe your formal knowledge (e.g. physiology, psychology, communication skills), previous nursing experiences with a similar problem, and/or personal experiences that helped guide you as you worked with the patient.

Noticing: What did you notice about the situation initially?  Describe what you noticed as you spent more time with the patient and/or family.

Interpreting: Describe what you thought about the situation (e.g. its cause, potential resolutions, patterns you noticed).  Describe any similar situations you have encountered in practice before. Describe any similarities and differences you observed when compared with the current situation.  What other information (e.g. assessment data, evidence) did you decide you needed as you considered the situation? How did you obtain this information? What help with the problem did you get from your preceptor? Your conclusion: What did your observations and data interpretation lead you to believe? How did they support your response to the situation? Include pertinent pathophysiology and/or psychopathology.

Responding:  After considering the situation, what was your goal for the patient, family and/or staff? What was your nursing response, or what interventions did you do? List all actions that you took. Describe stresses you experienced as you responded to the patient or others involved in the situation.

Reflection-in-Action and Clinical Learning:  What happened? How did the patient, family, and/or staff respond? What did you do next? What went well? Describe three ways your nursing care skills expanded during this experience. Name three things you might do differently if you encounter this kind of situation again. What additional knowledge, information, and skills do you need when encountering this kind of similar situation in the future? Describe any changes in your values or feelings as a result of this experience.

Summarize the topic and the different opinions that you found during your research. Is there consensus to the opinions or are there different views about the topic’s future? Discuss the impact of these potential outcomes on either health care quality, health care costs, or the status of health care professionals.

The Future of Health Care

Discuss Evidence-Based Practice in Health Care Delivery and research opinions about how it will be shaped in the future. Include at least two scholarly articles in your research.

Address the following prompts:

Write a page paper that explores the sentinel event, analysis of causes, develops a plan of corrective action, and proposes future evaluation of the plan. Conduct a literature review that informs a plan for corrective action.

Sentinel event

Instructions:
Research Sentinel Events in nursing, including but not limited to The Joint Commission Websites:
The Joint Commission (Links to an external site.)

Select a specific sentinel event that has taken place and is documented on The Joint Commission site. Your account of the event should be as realistic as possible, as if it actually happened in your hospital or health care setting – OR – choose one from your own experience and describe the event as it happened or likely happened. As the department leader, you will be responsible for creating a fishbone diagram & a plan of corrective action to satisfy The Joint Commission, and to prevent future sentinel events.
