This assessment will provide you with the skills and knowledge required as part of the background work prior to a comprehensive primary health care initiative. You will learn how to describe a health problem and write a health problem statement, which is a key task to assist with focusing any health initiative.

What do you do? What is the ethical approach in this situation? How do you treat the community knowing that you may have to watch some die while you reserve treatment for specific individuals?

The Ethics of Triage

You are a nurse in a Third World country in a small village of 150 people (adults and children – roughly equal in numbers). There is an outbreak of a deadly infection which is easily treated with a specific antibiotic. Untreated, the mortality rate is close to 100%. However, you only have enough to treat 25 adults or 50 children. Additional dosages are on their way but are not expected to arrive before the disease will run its course.

Note that anyone over 11 is considered an adult and you are in as much danger from the infection as everyone else.

What do you do? What is the ethical approach in this situation? How do you treat the community knowing that you may have to watch some die while you reserve treatment for specific individuals?

EBP Guideline/Position Statement Paper:

Poor management of clinical depression within the oncologic population negatively impacts patient outcomes and quality of life.

APNs must determine the strength of the recommendations and guidelines to regulate whether policy changes are warranted. Through this process the APN may begin with a clinical question in mind before searching the literature on the proposed changes needed.

Step 1: Introduction
A good introduction grabbing the reader’s attention providing insight to the POI and significance to the APN’s role. Need strong thesis or purpose statement.

Step 2: POI Significance:
Discuss why this POI is significant to your role as an APN. How will it change practice? Why did you select this topic? Next, discuss all 3 measures: structural, process and outcomes by comparing them to one another or identifying how/what is different. What expected structural, process OR outcome measures are you hoping to see with instituting a change in practice? You must clearly identify the measure you are evaluating in this example and it should relate directly to your POI.

Step 3 PICO :
You will include a PICO framework AND the clinical question in this section. Follow these directions as specified.

Outline the PICO framework by listing (do not write in a paragraph format) P-population or patients, I-intervention, C-comparison, and O-outcomes as it relates to your POI. Then develop the clinical question by including all parts of the PICO framework in the question. Be as specific as possible in both the PICO framework development and clinical question and avoid phrases such as: “compared to the standard practice” , or “compared to the current practice” because faculty do not know what the normal practice or standard practice is at your facility..

Critique the guideline or position statement by discussing ALL of the following and including appropriate support from your guideline/statement selection:

(1) purpose/aim of the recommendation/postition statement-are they clear, current and relevant to your population?

(2) Recommendations or objectives of the guideline/statement

(3) Who (authors, groups, organizations) developed the guideline/position statement and what biases or funding did they receive? Did the authors, groups or organization have a valued interest in the guideline, why or why not? Remember to consider the author’s role professionally as part of your answer.

(4) Describe if they information is credible and justify your answer. What makes information credible? Is their truth in the data, interventions recommended, or studies used to support recommendations, how do you know?

(5) What levels of evidence were used to support each recommendation statement-look at the studies and resources used to develop the guideline or position statement. What levels of evidence were these resoruces/studies? Are those individual studies/resources of strong or weak on the evidence hierarchy? This discussion is to look at, analyze and draw conclusion as a  professional nurse looks at the data available to our patients.  Some of you may be familiar with Value Based Purchasing..or ‘pay for performance’.  Care such as the quality bundles for ventilator patients or hospital acquired infections fall under this work and this get reported publically.  Also one the Aims is the patient experience.  There are many questions about how the patient perceives their experience.

Patient 1

Mr B has paraplegia from last 10 years. He has muscular contractures and is total care. He has been admitted for a week with diagnosis of left leg cellulitis. Mr B has family members that stay with him during the day and night. Mr B. is receiving IV antibiotics for his cellulitis. Mr B. is also scheduled to have daily dressing for his left leg cellulitis. MR B needs turning and repositioning every 2 hrs and as needed. His vital signs have been stable. The plan is to discharge him with PO antibiotics once the course of IV antibiotics is done. The patient is otherwise medically stable.

Patient 2

Mr C is just admitted from emergency today with diagnosis of acute heart failure and lower GI bleed. The Hgb is 75, the patient is ordered to have 2 units of PRBC. The patient gets SOB on minimal exertion and is on 5 L of oxygen. The oxygen saturations are between 94-95% on 5 L of oxygen. The patient is very anxious and is calling for help often.

The charge nurse assigns Mr. C to RPN Melissa RPN and Mr. B to Shana RN. The charge nurse thinks that Melissa has dealt with more complex situations in the past and is familiar with the routine and possesses more knowledge, skill and judgment.
