Context Healthcare is a dynamic endeavour which means those providing care work in state of near constant change, be that in relation to the environment in which care is delivered, the nature of care interventions, equipment or drugs used to treat patients, and the pathways and process designed to ensure efficacy in the organisation of care delivery; all of which culminate in the outcome’s patients experience.
Large organisations often have a team of people dedicated to supporting service quality improvement, but even if you work in a place with such a team it would be wrong to think that quality improvement is their sole responsibility. Whilst organisations can facilitate change, Improvements in care quality take place where care is delivered, by the people delivering it.
As W. Edwards Deming said “Quality is everyone’s responsibility” The task It is likely that any reflections you have written previously have focused on a specific situation or incident you encountered at work. Whilst your personal experiences are still a key aspect of how you address this topic, you might also want to think more broadly about the contribution of the ‘Nursing Associates role’ to improving care quality. In reflecting upon the role of Nursing Associates in relation to improving care quality you might consider the following:
 What ‘Improving care quality’ means. What are the key elements of quality? You could use these to structure the body of your essay.
 How do you personally ensure the care you provide (or are part of providing), is safe, effective and leads to a positive patient/client experience?
 How do you/could you support others to improve the quality of the care they provide?
 What impact does the nature of the clinical/care environment and the culture, the ‘attitude’ towards change, have on efforts to improve care quality?
 What contribution can Nursing Associates make as part of a multi-disciplinary team (MDT) in relation to improving care quality? Why is an MDT approach beneficial?
 Wherever you can, illustrate your account with examples from your practice including what was good or not and why; what you might do differently in future. PRESSURE ULCERS

1) Analyze a healthcare organization to determine the key characteristics (including organizational values, if applicable) impacting its quality of care, with a focus on the relationship between quality outcomes and healthcare reimbursement.

Analyze Henry J. Carter Specialty Hospital and Nursing Facility, they are a Long Term Acute Care Hospital in NYC . The population consists of critically ill patients, some requiring prolonged mechanical ventilation. As a result of immobility, patients inevitably develop pressure ulcers. Pressure Ulcers (HAPU) are very common. According to the Agency for Healthcare Research and Quality, more than 2.5 million people in the United States develop pressure ulcers each year (2021). The good news is that with adequate treatment pressure ulcers can improve and heal.

3) Articulate a proposed initiative, including recommendations of interprofessional strategies to improve the patient care problem.

It is critical to make quality improvement a key component of healthcare organizations to provide patients with safe, high-quality, and timely services (Drew & Pandit, 2020). Senior executives must serve as role models. Whether they realize it or not, their actions are echoed throughout the organizations they govern. Staff will evaluate what is essential based on where and how leaders spend their time, not what they say. Management, leadership, and quality assurance are three separate but intertwined concepts (Drew & Pandit, 2020). Some managers are not necessarily leaders and vice versa. Some leaders and managers, but not all, will engage in QI, which can be done independently of leadership and management. However, combining all three is likely to produce the best results. Management entails commanding and controlling a group or team to achieve a common goal in its broadest sense. Leadership entails persuading others to help you achieve your goals. Management necessitates keeping track of details, responding promptly, and issuing orders or instructions when performance falls short of expectations, and QI frequently necessitates a purposeful relaxation of these standards. One may argue that QI necessitates more individuals acting as leaders and fewer acting as managers. However, many of the functions and responsibilities of management are shared among well-functioning, trustworthy frontline teams in the most extreme variants of QI (Drew & Pandit, 2020).

Systems leadership is becoming a global approach to public health issues “where emphasis is placed on the relationships between the parts that forma a physical system in addition to understanding the individual parts and their environment separately” (Bigland, et.al., 2020, p. 2). It is important to examine and work to improve all facets of a situation/issue to establish a continuous system of quality improvement. In order to move forward to continuously elevate work and improve quality, all parts of the system must work together and it’s important to have strong leadership that “shifts attention to leadership beyond organizational boundaries and across the wider network of groups, organizations, communities and stakeholders involved in creating and sustaining whole systems change” (Bolden, et.al., 2019, p. 28).
