Page length: 7-10 pages, excluding title page and references.
APA format current edition
Include scholarly in-text references throughout and a reference list.
Include at least one table that the student creates to present information.
REFERENCES MUST BE CURRENT WITHIN LAST 5 YEARS
NEEDS to include:
1. Introduction (Identification of the problem) with a clear presentation of the problem as well as the significance and a scholarly overview of the paper’s content. No heading is used for the Introduction per APA current edition.
2. Background and Significance of the disease, to include: Definition, description, signs and symptoms, and current incidence and/or prevalence statistics by state with a comparison to national statistics pertaining to the disease.  Create a table of incidence or prevalence rates by your geographic county/city or state with a comparison to national statistics. Use the APA text for formatting guidelines (tables). This is a table that you create using relevant data, it should not be a table from another source using copy/paste.
3. Surveillance and Reporting: Current surveillance methods and mandated reporting processes as related to the chronic health condition chosen should be specific.
4. Epidemiological Analysis: Conduct a descriptive epidemiology analysis of the health condition. Be sure to include all of the 5 W’s: What, Who, Where, When, Why. Use details associated with all of the W’s, such as the “Who” which should include an analysis of the determinants of health. Include costs (both financial and social) associated with the disease or problem.
5. Screening and Guidelines: Review how the disease is diagnosed and current national standards (guidelines). Pick one screening test (review Week 2 Discussion Board) and review its sensitivity, specificity, predictive value, and cost.
6. Plan: Integrating evidence, provide a plan of how a nurse practitioner will address this chronic health condition after graduation. Provide three specific interventions that are based on the evidence and include how you will measure outcomes (how will you know that the interventions have utility, are useful?) Note:  Consider primary, secondary, and tertiary interventions as well as the integration of health policy advocacy efforts. All interventions should be based on evidence – connected to a resource such as a scholarly piece of research.
7. Summary/Conclusion: Conclude in a clear manner with a brief overview of the keys points from each section of the paper utilizing integration of resources.
The paper should be formatted and organized into the following sections which focus on the chosen chronic health condition.

Identify two assessment tools to use with Gloria.

Gloria Oaks is a 66-year-old divorced Caucasian woman who has lived alone for about two years. Although Gloria was reluctant to seek any help, her adult children have convinced her to come see you. Gloria has not cleaned her home or taken care of herself for months. Her son Jeremy and her friend Jeanne come with her to the first appointment.

Jeanne, Gloria’s only good friend, calls her every day. Friends since high school, Jeanne is no longer able to visit Gloria because there’s too much stuff in the house. Jeanne says Gloria’s problems really started after her parents and her younger brother were killed in a car crash during their senior year of high school. After Gloria’s family died, she lived with an aunt and uncle for a year before heading off to college. Gloria married after her first year of college and the couple soon had two children. Gloria and her husband divorced after 47 years of marriage.

Gloria continues to live in the family home. Jeremy states that he and his sister had tried to help her clean up a few times but their mother continues to bring more and more stuff into the house. He states he believes his mother is depressed, is not sleeping, and is starting to experience some health problems from not leaving the house.

In your initial post, select two of the following questions to answer from your perspective as her psychiatric nurse practitioner.

In your initial interview with Gloria, Jeremy, and Jeanne, what types of questions would be most important to ask?
Identify two assessment tools to use with Gloria. Provide a rationale.
Identify three of your most important safety concerns for Gloria. Provide a rationale.
Identify resources (including links) available in your own community to help Gloria.
If you were to offer Gloria psychotherapy, articulate a possible case formulation for working with her.

Explain how you would approach a family who wants “everything” done for a patient with only a limited time to live

Post an explanation of the role of the advanced practice nurse in facilitating the discussion of end-of-life care with patients and their families. Explain how you would approach a family who wants “everything” done for a patient with only a limited time to live. Then, explain when it is appropriate to involve hospice and how to approach patients and/or families who refuse hospice services. Finally, explain potential outcomes of the patient in the case study you selected and how you would facilitate the discussion of end-of-life care with this patient’s family.

Case Study 3
Mr. Marley, age 91, is admitted to the intensive care unit following a stroke. The stroke progressed from mild hemiparesis and difficulty speaking to complete unresponsiveness and an inability to swallow. His daughter feels certain, based on prior explicit conversations with her father, that he would not want to have any treatment that would prolong his life and leave him in a severely disabled state. Mr. Marley’s oldest son disagrees with his sister’s assessment of their father. The son claims that their father still has a strong desire to live, and that he has been very active in his church until this stroke. Because Mr. Marley cannot swallow, he cannot be fed. The family is asked about insertion of a feeding tube. It is explained to the family that without food and fluids, their father will die fairly quickly. There are no existing advance directives or a designated health care decision-maker noted for Mr. Marley.

Write a 4-5 page evidence-based proposal to support the need for a nurse informaticist in an organization who would focus on improving health care outcomes.

Write a 4-5 page evidence-based proposal to support the need for a nurse informaticist in an organization who would focus on improving health care outcomes.

As you begin to prepare this assessment, you are encouraged to complete the Team Perspectives of the Nurse Informaticist activity. Completion of this will help you succeed with the assessment as you explore the nurse informaticist’s role from the different perspectives of the health care team. Completing activities is also a way to demonstrate engagement.

Nurses at the baccalaureate level in all practice areas are involved in nursing informatics through interaction with information management and patient care technologies. Nurses must not only demonstrate knowledge of and skills in health information and patient care technologies, but also how to use these tools at the bedside and organizational levels. Moreover, nurses need to recognize how information gathered from various health information sources can impact decision making at the national and state regulatory levels.

Demonstration of Proficiency

By successfully completing this assessment, you will demonstrate your proficiency in the course competencies through the following assessment scoring guide criteria:

  • Competency 1: Describe nurses’ and the interdisciplinary team’s role in informatics with a focus on electronic health information and patient care technology to support decision making.
    • Define nursing informatics and the role of the nurse informaticist.
