Review research that addresses the issue/stressor and write a white paper to your organization’s leadership that addresses the issue/stressor you selected.

The Quadruple Aim provides broad categories of goals to pursue to maintain and improve healthcare. Within each goal are many issues that, if addressed successfully, may have a positive impact on outcomes. For example, healthcare leaders are being tasked to shift from an emphasis on disease management often provided in an acute care setting to health promotion and disease prevention delivered in primary care settings. Efforts in this area can have significant positive impacts by reducing the need for primary healthcare and by reducing the stress on the healthcare system.

Changes in the industry only serve to stress what has always been true; namely, that the healthcare field has always faced significant challenges, and that goals to improve healthcare will always involve multiple stakeholders. This should not seem surprising given the circumstances. Indeed, when a growing population needs care, there are factors involved such as the demands of providing that care and the rising costs associated with healthcare. Generally, it is not surprising that the field of healthcare is an industry facing multifaceted issues that evolve over time.

In this module’s Discussion, you reviewed some healthcare issues/stressors and selected one for further review.

For this Assignment, you will consider in more detail the healthcare issue/stressor you selected. You will also review research that addresses the issue/stressor and write a white paper to your organization’s leadership that addresses the issue/stressor you selected.

  • Review the national healthcare issues/stressors presented in the Resources and reflect on the national healthcare issue/stressor you selected for study.
  • Reflect on the feedback you received from your colleagues on your Discussion post for the national healthcare issue/stressor you selected.
  • Identify and review two additional scholarly resources (not included in the Resources for this module) that focus on change strategies implemented by healthcare organizations to address your selected national healthcare issue/stressor.

Develop a 4-page paper, written to your organization’s leadership team, addressing your selected national healthcare issue/stressor and how it is impacting your work setting.

Analyze strengths and weaknesses of leader in all five domains (communication and relationship management, knowledge of health care environment, leadership, professionalism, and business skills).

Contemplate the effectiveness of a nurse leader in a formal leadership role (charge nurse, director of nursing, nursing manager, administrator, or nursing instructor). This nurse could be a current manager, or a leader you identify from a past encounter. From the list, select “Nurse Executive Competencies.”
Write a three to four page paper (in addition to cover page and reference page) that analyzes the effectiveness of this leader in relation to all five of the AONE domains.
Include the following criteria when writing this paper:
Describe the nurse selected, including the title and scope of their current leadership role.
Analyze strengths and weaknesses of leader in all five domains (communication and relationship management, knowledge of health care environment, leadership, professionalism, and business skills). The “domain” is the large category, i.e. leadership.
Analyze how the leader you identified met or did not meet the specific characteristics listed under the domain. Make certain each of the specific characteristics under your chosen domain is addressed in your analysis. Use at least two competencies from each domain to describe the leadership of your chosen leader. The competency is the sub-category under a given domain, i.e. for leadership: systems thinking and change management. You may use any of the bullet points under the competency as part of your analysis.
Validate your analysis with personal experience and a minimum of three current scholarly sources. The AONE document should be cited and referenced. Your textbook, Leading and Managing in Nursing, may be considered as another scholarly source used for this assignment.

Does regular oral care reduce the incidence of ventilator associated pneumonia in critically ill adult patients?

There are three case scenarios in this discussion board.Choose whichever one you’d like, for your initial post.

After you’ve chosen your case scenario -find TWO articles that help to answer the clinical question. In two paragraphs, discuss the evidence in relation to the question. Post the article link or in PDF form. What do the articles say? How do they help answer the question? How do they each answer the question? Are the articles conflicting or do they come to the same conclusion? What strengths/limitations are there? Use your textbook to support whether the evidence is strong or not. What remaining questions do you have?

The textbook used in class is
Hoffman, T., Bennet, S., & Del Mar, C. (2017). 3nd ed. Evidence-Based Practice: Across the health professions. Australia: Churchill Livingston Elsevier.

You are the nurse in charge of a busy intensive care unit at a regional general hospital. An infection control audit has identified a high number of patients diagnosed with ventilator associated pneumonia (VAP) on your unit during the previous 12 months. You recently attended a critical care conference and heard a number of nurses talking about the role of oral care in preventing VAP and wonder whether an oral care policy would reduce the incidence of VAP in critically ill ventilated patients on your unit. You decide to search for research to answer your question with the aim of presenting the findings to the staff at the next unit meeting.

Does regular oral care reduce the incidence of ventilator associated pneumonia in critically ill adult patients?

Identify ethical issues common to research involving human subjects. Evaluate published nursing research for credibility and clinical significance related to evidence-based practice.

There are five main topics and the accompanying studies identified under the Evidence Based Practice tab. They are as follows:

1. Evaluating Nutritional Status a. Formative evaluation of the feedback component of Children’s and Adolescents’ Nutrition Assessment and Advice on the Web (CANAA-W) among parents of schoolchildren b. Validation of the Diet Quality Index for adolescents by comparison with biomarkers, nutrient and food intakes: the HELENA study

2. Identifying those at risk for malnutrition a. Beyond malnutrition screening: Appropriate methods to guide nutrition care for aged care residents b. Population-specific short-form mini nutritional assessment with body mass index or calf circumference can predict risk of malnutrition in community-living or institutionalized elderly people in Taiwan c. Comparison of the prevalence of malnutrition diagnosis in head and neck, gastrointestinal and lung cancer patients by three classification methods

3. Malnutrition associated with specific health conditions a. Malnutrition in a sample of community-dwelling people with Parkinson’s disease b. Nutritional risk index as a predictor of postoperative wound complications after gastrectomy c. Risk of malnutrition is associated with mental health symptoms in community living elderly men and women: The Tromso Study

4. Outcomes associated with nutritional status a. Role of nutritional status in predicting quality of life outcomes in cancer – a systematic review of the literature b. Nutritional support for liver disease

5. Interventions to improve nutritional status a. Interventions to enhance adherence to dietary advice for preventing and managing chronic diseases in adults Please choose one of the topics and identify one study under that topic. Read the study and answer one of the following questions, according to the topic chosen, in a 2-3 page (not including title and reference page) summary. Topic 1 – What methods can be used to assess nutritional status? Topic 2- What methods can be used to identify those at risk for malnutrition? Topic 3- What specific health conditions increase the risk of malnutrition? Topic 4- What associations exist between nutritional status and health outcomes? Topic 5- What type of interventions improve adherence to recommendations on nutritional intake?

Identify strategies the manager could use to resolve the conflict between the team members to create a supportive climate again. Explain how the defensive climate created by the conflict will affect the workplace relationship between team members

The patient access staff at an internal medicine practice are discussing a significant trend of no-show and canceled appointments. One receptionist stated that she was told by the coordinator to never schedule patients who call for same day appointments unless it is an emergency, such as medication reactions, so patients would value their appointments and discourage them from not showing up or canceling. The coordinator also indicated that the change was approved by the CEO. A receptionist became emotional and stated, “This is not right. We are here to help patients. Your plan does not seem to be working, and I think we should try something else.”

The coordinator then asked the staff how the patient recall list and the confirmation of appointments were managed. At this point, another receptionist indicated she thought the patient contact software was not working correctly. It was displaying confirmation icons for scheduled patients, but a few patients called to confirm their appointments on their own. She said, “We need to do something about this now and stop talking about it. I reported my suspicion several times to management but nothing was checked. I think patients calling to confirm their appointments is proof that the software does not work correctly.”

The coordinator continued to ignore the direct statements of the staff. Instead, she suggested she would study the relationship between patient gender, age, payer, and date appointments were set to determine if there were any patterns. “It’s about time you become involved in the solution,” a scheduler said. “Okay,” the coordinator said, “I am trying to solve this dilemma. We will accomplish nothing by complaining.” The first receptionist replied, “We will never solve the problem if you do not process our input.”

The coordinator smiled politely and nodded in agreement. In addition, everyone agreed that the data she would study could be helpful as a starting point. The coordinator further indicated she would have IT investigate issues with the patient contact software. A few days later, the coordinator met with all of the staff and shared her findings. Her finding revealed there was no relationship between date scheduled and canceled appointments or no-shows. She also indicated that IT found 893 patient portal messages unopened. The messages included requests to reschedule and text replies to cancel.

The group developed a consensus to follow the original scheduling protocol, which was patients are scheduled on a first call basis and the artificial barrier of two weeks was terminated. The coordinator accepted responsibility for the lack of follow-up on patient messages, but then assigned another scheduler the daily task of checking and responding to messages. The coordinator thanked everyone for their feedback, ideas, and support. She quipped, “Emotions are okay. Passion should be evident in all of us every day.”
