Evaluation of Plan Effectiveness Criteria

Over the past nine weeks, you selected an aggregate and conducted a risk assessment of its health, developed a care plan to address those health risks, planned to implement one intervention in a small group from the aggregate, and considered the effectiveness of the intervention on the health of the small group. It is time now for you to present your final submission of this Capstone project.

There are two parts to this assignment.

Part 1: A 10-15 slide power point presentation addressing the following:

  • A detailed description of the aggregate including strengths and weaknesses
  • A risk assessment of the aggregate
  • Diagnoses based on the risk assessment
  • A detailed care plan for the aggregate
  • A description of at least one intervention that was implemented (or the implementation itself if presented as a power point)

Part 2: In a Microsoft Word document of 3-4 pages formatted in APA style, you will describe the evaluation process of the intervention(s) you implemented.

Include the following in your paper:

An evaluation of the effectiveness of the intervention:

  • Did you implement the intervention as planned? If not, what were the reasons?
  • Interview/measure outcomes for two or three members of your group to determine/measure the effectiveness of the intervention(s).
    • Include transcripts of the interviews with the participants from the group in the appendix of your paper.
  • Finally, compare the projected effectiveness of your plan before implementation with the actual effectiveness after implementation.
    • Are there visible signs of success (for example, reduced health issues)? If yes, describe. If no, describe what signs you would anticipate and rationale for no change.
  • You may want to use the responses to the above two criteria to measure the actual effectiveness of the intervention.

You are asked to see Mr. Basset, who is 80 years old and has recently lost his wife. His children do not live nearby, but Mr. Basset sees them on holidays. Mr. Basset has had an unexplained weight loss of 20 pounds in 3 months. He is taking medication for hypertension and arthritis.
Mr. Basset reports that his wife did most of the cooking and he has limited cooking skills. He usually has cereal with milk and coffee for breakfast and soup for a second meal later in the day. He eats crackers throughout the day if he is hungry. In addition, Mr. Basset has ill-fitting dentures and claims that food just does not taste the same. He is also on a limited budget. More than anything, however, Mr. Basset is lonely.

Thus, healthcare providers must take into account the physical, emotional, social, and spiritual needs of patients in order to provide the most effective care (Langberg et al., 2019). Consequently, nurses can help patients to understand the impact that their lifestyle choices have on their health, and provide guidance on ways to make positive changes.

The caring theory addresses the concept of personhood through the idea of presence. Presence refers to the ability of the healthcare provider to be fully engaged with the patient both physically and emotionally. This means being fully present in the moment, attentive to the patient’s needs and concerns, and willing to listen and respond to their questions and feedback. This deep level of engagement is essential in building trust and rapport with patients, and can have a positive impact on their overall wellbeing.

The element of spirituality in healthcare cannot be underestimated. This can include religious beliefs and practices, as well as other forms of spiritual expression. By taking a holistic approach to healthcare, nurses can help patients to explore their spirituality and use this as a means of coping with illness and other challenges (Wei et al., 2019). The approach can help patients to find meaning and purpose in their lives, which can be an important factor in overall health and wellbeing. Healthcare providers must take a proactive approach to learning about the needs and preferences of individual patients. This can involve asking open-ended questions, actively listening to feedback, and working collaboratively with patients to develop treatment plans that take into account their unique needs and circumstances.

In conclusion, the nursing as caring theory places a strong emphasis on the concept of personhood, recognizing the unique qualities and characteristics that make each individual who they are. By taking a holistic approach to healthcare and recognizing the interconnectedness of all living things, nurses can provide more effective care and help patients to achieve better health outcomes. To effectively incorporate the concept of personhood into their nursing practice, healthcare providers must take a proactive approach to learning about and understanding the needs and preferences of individual patients, and work collaboratively with them to develop treatment plans that address their unique needs and circumstances.
