Which test(s) should be performed to determine whether the anemia is related to chronic disease or iron deficiency, and what would those results show? Should the practitioner consider a blood transfusion for this patient? Explain your answer.

The 50-Year-Old Patient Evaluation & Management Plan

A 50-year-old woman presents to the office with complaints of excessive fatigue and shortness of breath after activity, which is abnormal for her. The woman has a history of congestive heart failure with decreased kidney function within the last year. The woman appears unusually tired and slightly pale. Additional history and examination rules out worsening heart failure, acute illness, and worsening kidney disease. The CBC results indicate hemoglobin is 9.5 g/dL, which is a new finding, and the hematocrit is 29%. Previous hemoglobin levels have been 11 to 13g/dL. The patient’s vital signs are temperature 98.7°F, heart rate 92 bpm, respirations 28 breaths per minute, and blood pressure 138/72. The practitioner suspects the low hemoglobin level is related to the decline in kidney function and begins to address treatment related to the condition. Discuss the following:

  1. Which test(s) should be performed to determine whether the anemia is related to chronic disease or iron deficiency, and what would those results show?
  2. Should the practitioner consider a blood transfusion for this patient? Explain your answer.
  3. Which medication(s) should be considered for this patient?
  4. What considerations should the practitioner include in the care of the patient if erythropoietic agents are used for treatment?
  5. What follow-up should the practitioner recommend for the patient?

Based on this case study how would the nurse actualize Parse’s theory of Human Becoming? What are characteristics of a human becoming nurse? What are strengths and weaknesses to this theory of nursing?

Write a 1250-1500 word APA paper addressing each of the following points. Be sure to completely answer all the questions for each bullet point. Separate each section in your paper with a clear heading that allows your professor to know which bullet you are addressing in that section of your paper. Support your ideas with at least two (2) sources and your textbook using citations in your essay. Make sure to cite using the APA writing style for the essay. The cover page and reference page in the correct APA do not count toward the minimum word amount. Review the rubric criteria for this assignment.

Case Study

The hospice nurse sat with Ann’s husband, Ben. Ann was resting quietly as the increased dosage of IV pain medication gradually reached its therapeutic level. Ben turned his head and slowly turned, looking out the room’s only window. As he glanced up, a small flicker of light caught his breath. It was a shooting star. A tear fell from the corner of his eye and he turned to Ann. The nurse sensed that something significant to Ann and Ben was unfolding. Shuffling to Ann’s bedside, he took her small fragile hand in his. These hands had rocked cradles, burped babies, and groomed the horses she loved to ride. Gently holding her hand, he turned to the nurse. “She would ride like the wind was chasing her.” Looking back to Ann his voice broke; choking back tears “Ann, Ann I saw Jessie…Jessie is calling.” Ben turned “Jessie was our daughter. She died having a baby that was too big. When she died it was a pitch-black night. Cold, so cold, the baby died too, a little boy, named him Abe, Jr. after Jessie’s husband. I took Ann outside so she could cry to God above and there in this dark sky we saw two falling stars…together…just falling. We knew it had to be Jessie and Abe…two angels to light up the night.” Ben turned back as a deep sigh escaped from Ann’s lips. A soft smile remained as she joined Jessie and Abe.

. Based on this case study how would the nurse actualize Parse’s theory of Human Becoming?
. What are characteristics of a human becoming nurse? What are strengths and weaknesses to this theory of nursing?
. What challenges exist for healthcare institutions to switch to this nursing approach?
. How might Parse’s understanding of transcendence guide the nurse, as Ann’s death became a reality to Ben?
. From the nursing theories we have discussed, what additional theory would you apply to this case study? Develop a plan of care to include both nursing theories (be specific and provide reasons)

Assignment 5

Types of Standards
Chapter 22 focuses on standard languages. Many different types of standards of importance are available to different aspects of health informatics. Review the following websites, and make a list of the different types of standards you were able to identify in addition to standard languages:

  1.  http://ushik.ahrq.gov/index_sdo.jsp?system=sdo&enableAsynchronousLoading=true
  2.  http://library.ahima.org/xpedio/groups/public/documents/ahima/bok1_000024.hcsp?dDocName=bok1_000024

Health Level Seven Standard
Exploring a variety of standards is one of the ways to gain an overview of the standards used in healthcare informatics. Health Level Seven (HL7) standard is an example of a set of communications rules (standards) that define the ways by which applications exchange data. Complete an Internet search on HL7 standards, and review your results to discover ways in which the HL7 standard is applied. Identify one or two areas where healthcare suppliers and customers use the HL7 standard.

Describe three reasons it is important to gather detailed and extensive information from any patient before you counsel him/her or make medication suggestions.

Historical information from patients

After studying Module 2: Lecture Materials & Resources, discuss the following:

It is very important for all mental health professionals to take very detailed and thorough historical information from their patients. This information should include an adequate social history, complete medical history, and a full mental status examination with a probable treatment plan.

  • Describe three reasons it is important to gather detailed and extensive information from any patient before you counsel him/her or make medication suggestions. Use evidence-based research to support your position.
  • Define malingering. Discuss two ways to differentiate between malingering and a DSM5 diagnosis. Use evidence-based research to support your position.

Submission Instructions:

  • Your initial post should be at least 500 words, formatted, and cited in current APA style with support from at least 2 academic sources. Your initial post is worth 8 points.

Reflect on your own healthcare organization and consider any steps your healthcare organization goes through when purchasing and implementing a new health information technology system.

Week 9 Discussion Post

The Inclusion of Nurses in the Systems Development Life Cycle

In the media introduction to this module, it was suggested that you as a nurse have an important role in the Systems Development Life Cycle (SDLC). With a focus on patient care and outcomes, nurses may not always see themselves as contributors to the development of new systems. However, as you may have observed in your own experience, exclusion of nurse contributions when implementing systems can have dire consequences.

In this Discussion, you will consider the role you might play in systems development and the ramifications of not being an active participant in systems development.

To Prepare:

  • Review the steps of the Systems Development Life Cycle (SDLC) as presented in the Resources.
  • Reflect on your own healthcare organization and consider any steps your healthcare organization goes through when purchasing and implementing a new health information technology system.
  • Consider what a nurse might contribute to decisions made at each stage of the SDLC when planning for new health information technology.

Post a description of what you believe to be the consequences of a healthcare organization not involving nurses in each stage of the SDLC when purchasing and implementing a new health information technology system. Provide specific examples of potential issues at each stage of the SDLC and explain how the inclusion of nurses may help address these issues. Then, explain whether you had any input in the selection and planning of new health information technology systems in your nursing practice or healthcare organization and explain potential impacts of being included or not in the decision-making process. Be specific and provide examples.
