Major depressive disorder

In this mid-term assignment, you will write a 7-page paper in which you choose a behavioral/mental health topic (Major depressive disorder), review scholarly literature related to that topic, and describe how you would engage with a patient on this topic in an integrated care setting.

The goal of this assignment is two-fold. The first is to increase your knowledge of a mental health issue or medical problem (and its psychosocial manifestation) commonly seen in the primary setting. The second part is to improve your skills in conducting interprofessional collaborative practice (assessment and intervention) to address this issue in primary care.

Begin by choosing a topic. The following is a suggested list of topics; your topic can be something not on this list, but it must be related to primary care:

Major depressive disorder

The paper should be 7 pages (not including title page or references) typed in Times New Roman 12-point font, double-spaced, and include one-inch margins. APA style citations and references are required. Do not include an abstract. No direct quotes allowed—paraphrase only.

The paper is expected to include references a minimum of five empirical and theoretical scholarly articles that are not on the syllabus. At least four of your outside sources should come from scholarly journals, not books or websites. The DSM does not count as one of your scholarly sources.

 

Part I:

Conduct a review of the scholarly literature over the past 5 years that (4 pages) that:

Discusses the issue as it relates to the primary or integrated care setting.
Identifies how social determinants of health (social and economic conditions) can impede care or magnify the issue.
Provides recommendations on assessment tools and interventions that are most effective for interprofessional primary care practice.

Part II:

Illustrate how you would intervene in the primary care setting by creating a 3-page script/dialogue between yourself and a patient on your chosen topic. You should start the script as if you are behavioral health provider in a primary care setting, and one of the PCPs has just conducted a warm handoff and asked you to meet with the patient. Create a script of a 15-minute session (3 pages). The script should incorporate dialog from both provider and patient. For example:

  • BEHAVIORAL HEALTH PROVIDER: How are you doing today? The PCP just said you’ve been dealing with some concerns. Can you tell me about them?
  • PATIENT: Yeah, I’ve been feeling…

Your script does not have to be based on your actual behavioral health practice in a primary care setting. In the dialogue you create, your script should represent the following:

What you would do if you were actually in that setting dealing with the specific issue that you discussed in your literature review
How you would address and recognize the patient’s intersectional identities, and to ensure any intervention you conduct acknowledges how socio-structural and/or environmental factors impede care or magnify mental and/or behavioral health concerns
How you would ensure your assessment and interventions foster resilience

ABORTION

Media Analysis: For this assignment, you are to investigate and compare media coverage of one issue from both mainstream and alternative sources. Since you will be comparing reporting across sources, it is probably easiest to address an issue that is currently hot such as the current war in Ukraine, conflict in the Middle East, Corporate corruption, gay marriage, abortion, or immigration issues.

Read at least one mainstream print article from a newspaper or magazine. You may use online sources such as the Washington Post, USA Today, nytimes.com, newsweek.com, time.com, and so on. Provide a link to the sources you use in your paper. I prefer that you use online sources. Why? When watching TV news, the images and language go by quickly. Thus, it is difficult to fully reflect on what you are viewing. However, if you can record and playback the news or get the transcripts from the program, I would be willing to allow you to use TV news sources. Please first talk to me before using television sources.

Next, investigate the same topic in at least two alternative print sources, ideally one on each side of the political spectrum. For example, you might choose articles from Mother Jones, The Nation, National Review, The American Conservative, or Z Magazine (in print or online). Please provide the link for these sources. A final tip an internet search can provide you with access to alternative news beyond those listed above.

Analysis: address these questions in your paper.

Case Study

Margaret (age 38), identifies as a Mexican American homemaker, is seeking services from a local service agency. Margaret tells the caseworker that she feels overwhelmed with her usual responsibilities with the additional pressures of the current COVID-19 pandemic. Additionally, she feels depressed, doesn’t sleep well or eat much, and is irritable much of the time. She has eliminated most of her social interactions and the only outside activity she feels comfortable doing is attending Sunday Mass with her family. Recently, Margaret’s father (77) a retired U.S. Navy Lieutenant, had a fall, and is no longer able to live alone. Margaret helped him move into a local assisted living, but is struggling to ensure his emotional needs are met. She was able to sign him up for food stamps to assist with the cost of food. Her partner Jenna (age 42) also identifies as Mexican American, is an architect who works long hours and often unwinds after work by enjoying a few drinks. Margaret communicates that Jenna’s drinking upsets her because her father was an alcoholic and she is concerned about the few times Jenna has picked up the kids after she had a few drinks. Jenna is passionate about her architecture work and was awarded the Yuma Architect of the year in 2019. Due to COVID-19 Jenna moved her architecture practice to her home and now all four family members are under the same roof for the bulk of the time together. In talking with the case worker, Jenna reports the line between family and work has been blurred due to COVID-19, her drinking has increased after work to destress. She reports her work load has increased since working from home and she has no energy left to engage with her children or partner at the end of the day. She says that when she tries to cut back drinking during special events or during Lent, she finds that she is irritable and craves alcohol. She believes that if she wanted to, she could probably cut back but feels alcohol helps her relax. Jenna reports she is concerned about Lisa’s inability to focus and her struggling grades.

They live with their 2 children Lisa (age 15) who is capable of caring for herself and helping around the house, but prefers to hang out with her friends and be involved in sports and other extracurricular activities; and their youngest son, David (age 9), who has Down’s Syndrome and needs assistance in self-care and day-to-day activities.

Lisa (15) is very socially active in extracurricular and athletic events. In school she fidgets and has a hard time sitting still. She also struggles with time management and tends to be disorganized. She chronically misplaces everyday objects like her backpack, mask and cell phone. She will often be late to volleyball practice and her friends comment on how forgetful she is. Although she wants her school work to be perfect, she is prone to making careless mistakes. The struggle for perfection makes starting a new assignment feel very stressful, leading her to procrastinate starting in the first place. As a consequence, she recently failed a big test, which has led to her acute fear of failing 10th grade, and not being eligible to play volleyball. She recently made the varsity volleyball team as a 10th grader.

David (9) is thriving in his current school environment. He has great support in his special education classroom and is mainstreamed for several classes. Margaret and Jenna keep a consistent schedule for him throughout his entire day that sets him up for success.

 

The case worker receives a call yesterday from Margaret that Jenna was pulled over for driving under the influence two blocks from their house. Jenna was on her way home with the kids. Jenna was arrested and Margaret was called to pick up the kids. As of today, Jenna is currently in jail and Margaret came home to a card on the door from child protective services. Jenna is facing an aggravated DUI because of her blood alcohol level and because both children were in the car.
