History taking in pediatric nursing

The following task is designed to develop your skills in identifying key information from an interview with a parent and synthesizing that information to create a health history for the child.

Scenario: You are a student nurse in a pediatric setting, observing a senior nurse, take a patient health history via a tele-health appointment. Your role is to hand over what was said and observed, in the interview, using the ISBAR framework (see reading list for link to article on ISBAR, and pediatric health history. Follow the instructions below to complete the assignment.

A. Document the most relevant information about Sunny’s health and health history from the video interview using the ISBAR format. That is, write a script for what you would hand over to the reviewing doctor. This information should include both the subjective information offered by the parent and child and also what you can observe from the video- objective health assessment.
