Martha Brown is a 27 year-old-woman who lives in the hills of northern Kentucky in a small log cabin that has no indoor plumbing. She lives with her husband and 6 small children. The public health nurse makes a home visit after three of the children have been diagnosed by the school nurse as having lice. While the nurse is explaining the use of Rid (lice treatment) to the mother she notes, that Mrs. Brown has a persistent cough that she has had for two years. The nurse notes the cough is productive, that Mrs. Brown looks emaciated, and her color is extremely ashen. She tires easily. Although health insurance is a benefit of her husband’s job in a nearby mine, Mrs. Brown’s children were born at home, and she has never had a complete physical. When asked the reason she has not gone to a nearby free clinic Mrs. Brown replies “Sickness is God’s will, and he will cure me if he wants to. Anyway my family comes first and I don’t have the time. Besides, doctors can’t be trusted. My Aunt Jane went once, and she died the next week.”

Ms Susie Jones, a 37 year-old-black American client who has been coming to the hospital’s out-patient clinic because she has brittle diabetes. Ms. Jones’s diabetes was diagnosed 6 months previously; however, the diabetes remains uncontrolled by insulin. The client was placed on a regimen of 40 units of lente insulin with 5 units of regular insulin at 7:00 AM and 4:00 PM. When Ms. Jones comes to the clinic today, she has an episode of “blacking out,” or an insulin reaction because she forgot to eat after taking her morning insulin. This morning, as frequently in the past, Ms. Jones is at least one hour late for her scheduled appointment. Community Intervention

Based on your group’s discussions, you have arranged for a community meeting to discuss the violence in the community. The following people come to the first meeting:

  • A teacher from the high school whose concern is that her students aren’t performing as well in school as they had been. When she has addressed this with them, they verbalize that increased violence makes it more difficult for them to get their work done.
    • A minister who is very involved with his community. Several of his congregants have lost family members and/or friends to recent violence.
  • A social worker from a community-based organization 10 miles away. He provides clinical outpatient services and has recently heard an increased concern about violence from his clients.
    • A teen who works in the local afterschool program who has become aware of the children attending the program feeling frightened to leave the program at the end of the day.
  • An ER doctor from the local hospital who has treated an increased number of patients who are victims of violence crimes.
    • The manager from a large department store who has seen an increase in thefts in his store.
  • A police officer who is very concerned about the increased violence in the community that he is witnessing firsthand.
    • A senior who resides in the assisted living community who is concerned for his safety.

Choose one of the meeting attendees and create an intervention plan to address his or her concerns. This plan can be very specific to address the potential solution that will help this meeting attendee and the community as well. (Referring to your macro practice book for this assignment will be helpful.)
