How might the specific coding conventions of the healthcare nomenclature vary, and how might this impact the accuracy of medical coding? Are there levels of subjectivity in the coding process?

You are the health information manager at Lower Mountain Hospital. An auditor recently visited the hospital and found multiple errors in your ICD-10 and CPT coding. Below are some examples of the errors the auditors identified:

  • In many instances, the “code first,” “use additional code,” and “code also” conventions for ICD-10 were not applied.
  • The level of detail in ICD-10 coding was not sufficient.
  • The CPT modifiers were used inappropriately, and procedures were unbundled unnecessarily.
  • The codes for the principal diagnoses (ICD-10) must directly relate to the treatment procedures (CPT) performed, and mistakes in alignment between the two were found.

As a result of the auditor’s findings, the board has asked that you evaluate Lower Mountain Hospital’s current coding processes and recommend approaches to improve coding accuracy. You will present your evaluation and plan to the board.

Directions.

As the health information manager at Lower Mountain Hospital, you will create a coding process improvement report to present to the board, in which you evaluate current processes.

  • Current Processes: Review medical coding practices to determine some common coding errors that may have contributed to the inaccuracies at Lower Mountain Hospital, provide reasons why these errors occur, and identify key regulations and coding conventions that must be followed to ensure accurate medical coding in healthcare organizations.
    • Describe some common coding errors that may have contributed to the inaccuracies at Lower Mountain Hospital, as well as the reasons for these inaccuracies. Consider the following:
      • How might the specific coding conventions of the healthcare nomenclature vary, and how might this impact the accuracy of medical coding?
      • Are there levels of subjectivity in the coding process?
      • How might organizational policies and resources, or the lack thereof, impact the coding process?
      • How might human error impact the accuracy of medical coding?
    • Describe key regulations that healthcare organizations must adhere to in order to ensure accurate medical coding, and describe the impact of these regulations on the coding process.
  • Recommendations: Now that you have determined some common coding errors that may be impacting the coding at Lower Mountain Hospital, as well as regulations that the organization must adhere to, you will now provide the board with some process improvement recommendations.
    • Recommend processes and practices Lower Mountain Hospital should follow to ensure accurate medical coding. Consider the following:
      • How might the organization use technology to enhance the accuracy of its coding?
      • How might the organization ensure that its coding team is using the most current coding conventions and all share the same understanding?
      • What organizational policies might you recommend to ensure accurate coding and support continuity of care, as well as adhere to relevant guidelines and regulations?
      • How can the organization ensure that it is prepared to address current and future trends related to reimbursement practices, quality-of-care initiatives, and data governance?
    • Explain the importance of improving processes and ensuring accurate medical coding, as well as the repercussions of coding inaccuracies.

What to Submit

Every project has a deliverable or deliverables, which are the files that must be submitted before your project can be assessed. For this project, you must submit the following:

Coding Process Improvement Report
Evaluate Lower Mountain Hospital’s coding processes, and recommend approaches for improving coding accuracy. Your report must be 500 to 1,000 words in length and cite any references.

Identify three crime prevention programs currently being utilized in communities and found in the K–12 school system, either national, state or local programs.

Assume the role of a social science researcher in charge of developing a new crime prevention program targeted toward juveniles in the K–12 school system. Review current crime prevention programs that target juveniles and adults. There are several crime prevention programs that are designed for adults and which are also in K–12 schools nationwide with a standardized curriculum (i.e., Neighborhood Watch, D.A.R.E. and G.R.E.A.T.) with many more programs created at the local level (i.e., Citizens Academy, Big Brothers/Big Sisters, and YMCA after school programs are unique to the community).
Identify three programs currently being utilized in communities and in the K–12 school system, national, state, or local programs. Compare and contrast the programs in terms of goals and objectives, target population (e.g., who is receiving and participating in the program), and how and by whom the programs are delivered (i.e., taught by law enforcement officers, school counselors, individuals outside of the school system, etc.). Based on these comparisons, explain which elements you would place in your program and how your program will be delivered to the students.
Create a proposal for a new crime prevention program that targets juveniles and determine how you will evaluate its effectiveness. Use elements of the three crime prevention programs you researched or come up with completely different components.
In the analysis of how your program will deter juvenile delinquency, you will want to use data from your three comparison programs to support your new program. For example, how many juveniles are enrolled in the program? In order to deliver the best programming possible for juveniles in the K–12 school system, calculate the optimal ratio of staff to the target population, based on the average ratio of the comparison programs.
Your analysis should specifically identify the data source and include a summary of the data.
Specifically address how the data was used to inform your thinking on the development of your new program (i.e., how you used the data to solve a real-world problem).

Create an 8 page word document to include the following:

• Identify three crime prevention programs currently being utilized in communities and found in the K–12 school system, either national, state or local programs.
• Tell about each program, explaining its goals, target population (i.e., adults, elementary school kids, 5th graders only, kids in each grade level, etc.), and how/where it is delivered.
• List and explain the pros and cons of each program.
• Compare each program to the others.
• Discuss the differences in methods used in the programs between youth and adults.
• Tell how successful you believe each program has been and justify your opinions.
• Explain which elements of each program you would include in your program and why you feel they are important.

• Who is your target population, what are your goals, and how will your program be implemented?
• Explain your program, detailing the elements of it and why it will be effective.
• Describe how the program’s effectiveness will be analyzed and how the objectives will be evaluated.
• Specify how deterrence will be measured and what will be considered effective deterrence.

Cultural Competence Practice Paper -The Cultural Competence Practice Paper is a compilation of course content covered and is the department’s Standardized Assignment for Cultural Diversity. In addition to a numeric grade, the assignment will be used to calculate scores for achievement of specific competencies. The instructor will provide students with a competency report before the end of the semester (see attached).
The purpose of this is assignment is for students to (1) examine the dynamics of oppression and discrimination, especially for populations at risk, (2) apply knowledge of cultural diversity to individual development and behavior, (3) appropriately intervene in an advocacy role geared toward social change, and (4) develop the ability to demonstrate an understanding of values of diversity, human rights, and social and economic justice, and (5) understand from the various system levels (micro, mezzo, macro). Students will complete this assignment using their selected cultural group from the Diller (2019) textbook (Figure 1) and topic selections (Figure 2). The assignment expands the student’s awareness of both the cognitive knowledge and skill necessary to effectively interact with and serve culturally diverse populations utilizing the social work cultural competence framework (refer to Lum, 2011, chapters 4-7). The cultural competence framework has three functions: awareness, knowledge, and skill.

Is there a profile for a manager who may struggle with the evaluation process? What are the key elements of a successful employee evaluation process? How should a manager prepare for the event?

Why Do Some Managers Hate the Performance Evaluation? Some managers would rather get a root canal than go through the performance evaluation process with their direct reports. Why? What does research tell you? As you respond to the two questions listed above, address the following:
• What traits does an effective manager display in the workplace?
• Is there a profile for a manager who may struggle with the evaluation process?
• What are the key elements of a successful employee evaluation process? How should a manager prepare for the event?
• Develop an evaluation form for your direct reports and show how the evaluation criterion correlates to the organization’s objectives.

Does the author make appropriate and accurate use of course concepts in constructing his or her discussion? (20%) Intangibles: Is the paper on the assigned topic? Is it the author’s own work?

Select just one topic.

1. Present one of the following two interactions: Kant and Anselm, or Descartes and Hobbes. This requires you to present both the argument from Anselm or Descartes and the criticism from Kant or Hobbes.

Evaluate the interaction. Is the critic’s objection a good philosophical objection? (You should have a rough idea what that means by now. If you do not, you’re in trouble.)

Given your evaluation, should Kant/Descartes have believed in God’s existence? Justify your answer. What does your answer mean for the rest of us? May anyone be justified in believing in God’s existence?

This is a 4 to 5 page paper, though you may certainly write more than that. Cite the text to support your claims. Write the paper is if it were a college paper. See the rubric for guidance.

2. Suppose someone argues for skepticism like this: People have different beliefs. People disagree about how old the world is; whether there is life on other planets; and even whether Bigfoot exists. So there’s really no truth about how old the world is; whether there’s life on other planets; or even whether Bigfoot exists.

Use the relevant course readings to evaluate the force of this argument. (You should be able to determine which are relevant.) Is this argument for skepticism a good one? What does your judgment with respect to that last question mean? Should we form beliefs based on arguments like this?

This is a 4 to 5 page paper, though you may certainly write more than that. Cite the text to support your claims. Write the paper is if it were a college paper. See the rubric for guidance.

3.Suppose someone tries to show that we should believe God exists like this: Any proposition is either true or false. Thus, there is a 50% chance that any proposition is true. Since “God exists” is a proposition, there is at least a 50% chance that God exists. If there’s a 50% chance that God exists, it makes sense to believe that he does. Thus, we should believe that God exists.

Put this argument in your paper. Then, explain whether it lives up to the standards of philosophy. If you say it does, explain why. If you say it does not, explain why.

This is a 4 to 5 page paper, though you may certainly write more than that. Cite the text to support your claims. Write the paper is if it were a college paper. See the rubric for guidance.

1. Does this paper identify have a clear thesis? (5%)
2. Does this paper contain only relevant information? Are the citations completed properly? (5%)

3. Does the paper attribute the correct view to the philosophers in question? (10%)

4. Is/are the philosopher’s view presented with the appropriate level of detail?
(For example, does the author explain concepts and arguments in a tight manner, or are the arguments and concepts merely sketched?)
(25%)

5. Does the author present a clear argument in his/her discussion? (15%)

6. Does the paper cohere? Or, is the paper a hodgepodge of disparate ideas? (10%)

7. Does the conclusion tie together the different phases of the paper? Or, is the conclusion a non-sequitur? (5%)

8. Are the spelling, grammar and syntax on the college level? (5%)

9. Does the author make appropriate and accurate use of course concepts in constructing his or her discussion? (20%) Intangibles: Is the paper on the assigned topic? Is it the author’s own work?
