You are working at a busy family practice group and you have a patient, ML, that is establishing care for the first time with your practice, and comes to you with the following scenario:

ML is a 54 y.o. Hispanic female with hx of chronic shoulder and back pain that began 10 years ago when she was in a boating accident

She lives in both US and Mexico, making regular visits across the border; lately, she has stayed in US due to Covid border crossing constraints, living with her daughter’s family

She had rotator cuff surgery in 2011 and reports to you that due to a long operation and poor positioning, she has suffered from not only pain, but also chronic numbness and tingling in her R shoulder

As “la abuela” (grandma) she is the primary caregiver of the children and homemaker for her family and her pain is exacerbated with housework, and especially with the prolonged carrying of her grandchildren; one of which is 10 m.o.

Currently, her med list is as follows

Losartan 50 mg BID for HTN

Gabapentin 300 mg po BID for pain

Atorvastatin 40 mg daily for cholesterol

Diazepam 5 mg po up to TID prn pain

Norco 5/325 mg – takes up to two, sometimes up to 4-5x a day, prn pain

She is a smoker, only smokes outside the house, and drinks 2-3 cans of beer on the weekends, but more on family celebrations

Denies recreational drugs and denies past overdoses

Recently moved to CA more permanently to stay to take care of children during Covid/school closures

She asks you to refill all her meds for 6 months like her doctor in Mexico did so she doesn’t have to make another co-pay and come back and see you so often; it’s hard for her to get an appointment, and with Covid her daughter has to take off of work to watch the kids just so she can come to you by bus since there is only one family car

Here VS are 135/75, 80, 97.5, 20 and PE unremarkable other than R shoulder exam with pain with ROM, but full ROM, no tenderness, otherwise normal, back exam including SLR are normal/neg

The former Sameer Shariff, a Saudi Arabian national who changed his name to “The Left Hand of God” and is known to his followers as “Hand,” is a suspected terrorist. Department of Homeland Security (DHS) agents believe that he is planning an imminent attack somewhere in Capital City. Under duress to prevent the attack, three DHS agents and three FBI agents storm into Hand’s house, burst into his bedroom, where he is in bed with a woman, and point shotguns at him. They demand to know what he is planning. No Miranda warnings are read. Hand tells them that he has hired Alex “Boom Boom” Jaxon, a known explosives expert, to plant a bomb in the Capital City Arena, with a timer to go off in three hours when the arena will be filled with over 20,000 people for a music concert.

Jaxon is known to the police and is in Central City, which is two hours away. He is arrested by Central City police and calls his lawyer in Capital City, who tells him he will meet him there and to remain silent. The lawyer then tells Central City police that they are not to question Jaxon until they arrive in Capital City and the lawyer is present.

Central City Police Officer McFadden drives Jaxon to Capital City. McFadden knows that DHS, FBI, and Capital City police are searching the arena, but McFadden is afraid they will not find the bomb in time. By the time that McFadden and Jaxon arrive in Central city, the concert hall is already full of audience members, and the show is set to begin in twenty minutes.

McFadden, who knows Jaxon’s family from previous police contacts, tells Jaxon that his mother and two brothers, who have been surveilled by the police for the last six weeks, are at the concert and will die if the bomb goes off. Jaxon leads McFadden to the bomb, which is defused. A crying Jaxon states that his purpose was to kill infidels, but he would never harm his family.

Research Fifth Amendment cases, including Miranda v. Arizona, 386 U.S. 436 (1966), which involves custodial interrogation and the right against self-incrimination.

Climate Change Act 2021 (fictitious)

The Climate Commission was created through the enactment of the Climate Change Act 2021 (fictitious). Section 5 of the Act gives the Climate Commission the power to fine any business operating within the United Kingdom that has a negative impact upon the government’s obligations under the Paris Agreement, where the greenhouse gas emissions for which they are responsible exceed an industry-based target. The Climate Change Act 2021 requires that the Secretary of State for the Environment approves any decision of the Climate Commission before issuing fines to any company.

Duff Beer is a brewery that operates within the UK brewing industry. The brewing industry has managed to keep within its agreed target for greenhouse gas emissions and Duff Beer have complied with their own targets for gas emissions. Moe Szyslak, the President of Duff Beer, learns that the Climate Commission intends to fine Duff Beer for failing to operate within their commitments for limiting the amount of gas emissions released into the air through their brewing operations.

Moe contacts the Climate Commission and sends them an independently audited report of Duff Beer’s greenhouse gas emissions. The report shows that Duff Beer is compliant and is operating within the targets. The report also includes an industry-wide report detailing that the whole brewing industry is compliant with the targets that have been set.

When the Climate Change Act 2021 was debated in Parliament, Moe appeared in television interviews to express his disagreement with the creation of the Climate Commission. He also organised several protests and petitions against the creation of the Climate Commission.

The Climate Commission’s decision to fine Duff Beer is authorised by the Secretary of State for the Environment following her meeting with the Prime Minister who reminds her that tighter control of greenhouse gas emissions was a fundamental principle of current government policy.

For this assignment propose a scenario where you or someone you know are confronted with a moral dilemma relating to cultural diversity and multiculturalism.

Cultural diversity refers to religious, sexual, racial, and other forms of social difference. A moral dilemma is a situation in which one must make a decision between two or more options such that the options involve seemingly ethical and/or unethical conduct. Address the following questions:

Coagulation

Jessie was a typical 2-year-old who liked to try and spread his wings. He would run and fall throughout the house. On several occasions, with minor falls, a bruise would develop on his legs or arms. Jessie’s parent would often worry about being reported for child abuse because of the easy bruising that Jessie experienced.

At three, Jessie would complain that his ankles hurt. This was especially true after a hectic day of running and playing with other kids at the playground. Jessie’s parents wondered if he was simply trying to get attention. Which he usually did being an only child.

When Jessie was four, he accidentally ran into a small tree, hitting his nose. The nose bled excessively. Jessie’s mother tried to stop the bleeding, first with paper towels, which Jessie soaked through quickly, and then with a cold washcloth. Nothing seemed to help. Nervous about the amount of blood that Jessie was losing, his parents rushed him to the emergency room.

The emergency doctor ordered some blood work that included a Prothrombin Time (PT)and a Thromboplastin Time (PTT) test for clotting deficiencies. The site of the venipuncture for the blood draw, also took some time to stop bleeding.

An hour after the blood draw, the physician returned to the ER room to discuss the test results. Jessie’s PT results were normal but his PTT results were abnormal with an extended clotting time. The PTT clotting time was much longer than normal indicating the Jessie may lack of or decreased levels of a clotting factor.
